Spiritual Care Coordinator - Hospice

We are seeking a compassionate, flexible Hospice Chaplain to join our dedicated interdisciplinary team! This is an opportunity for a caring and spiritually grounded professional to provide meaningful support to patients and families as they navigate the end-of-life journey.

The Hospice Chaplain is responsible for the provision of spiritual care services to patients, families and caregivers of Faith & Family Hospice, either directly or through coordination of care with other spiritual counselors, offering spiritual and emotional support based on each patient's individual beliefs, values, and wishes. Working alongside and collaboratively with our full-time Chaplain, Nurses, Social Workers, Home Health Aides, Physicians, Music Therapists, and leadership team, the Chaplain helps ensure that the whole person-and the whole family-is cared for.

• Hospice experience preferred, but a strong understanding of and commitment to end-of-life care is essential
