Job Description - Staff Pharmacist - Long Term Care Pharmacy
Position Summary

About the role
We are hiring a Long-Term Care (LTC) Pharmacist to join our growing closed-door pharmacy located in Wilmington, Delaware. As an LTC Pharmacist, you will play a vital role in ensuring safe, accurate, and timely medication management for residents in Assisted Living, Group Homes, Memory Care, and other LTC facilities.
Hours of Operation:
Monday–Friday: 9:30 AM – 6:30 PM
Saturday: 10:00 AM – 2:00 PM
This is an on-site position in New Castle County.
What you'll do
Medication Verification & Clinical Review
Perform final verification of prescriptions ensuring accuracy in medication, dosage, directions, and day supply.
Conduct DURs, allergy checks, drug interactions, and therapeutic duplication reviews.
Review MARs, provider notes, facility orders, and eMAR entries for clarity and compliance.
Verify cycle-fill batches (7-day, 14-day, and 28-day) and ensure accurate blister/strip packaging.
Clinical & Patient-Centered Care
Provide clinical recommendations to providers, nurses, and caregivers.
Review PRN and routine medications for appropriateness and resident safety.
Support high-risk medication monitoring (e.g., anticoagulants, insulin, psychiatric medications).
Assist with Consulting Pharmacist duties, including Medication Regimen Reviews (MRR).
Participate in care plan meetings, audits, and quality improvement initiatives.
Regulatory & Compliance
Ensure compliance with CMS, Medicare/Medicaid, DEA, and State Board of Pharmacy LTC regulations.
Maintain perpetual inventory for CII–CV medications and emergency kits.
Oversee processes for expired inventory, return-to-stock, waste documentation, and audit readiness.

Qualifications
Required
Doctor of Pharmacy (PharmD)
Active Delaware Pharmacist License (RPh or PharmD)
Strong communication skills with nurses, facilities, providers, and internal teams
Ability to work efficiently in a fast-paced LTC workflow with exceptional attention to detail
Experience using pharmacy management systems (SuiteRx, Framework, PioneerRX, QS1, or similar)
Preferred
1–3 years of LTC, hospital, consulting pharmacy, or geriatric care experience
Knowledge of Medicare Part A, Part B, and Part D billing processes
Location
This is an on-site position located in Wilmington, Delaware (New Castle County).
