Job Description - Stroke and Chest Pain Program Coordinator

DUTIES AND RESPONSIBILITIES:
Essential Job Duties:
Provides coordination for the organization of services and systems necessary for a collaborative approach to stroke or chest pain care.
Assumes the day-to-day responsibilities of process and performance improvement activities as they relate to nursing, and ancillary personnel and assist in carrying out the same functions for all staff.
Plans, implements and evaluates clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care and nursing practice in stroke/chest pain care.
Responsible for the Stroke/Chest Pain Program development, quality assessment, education, improvement and inter/intra disciplinary communications.
Works collaboratively with all hospital and clinic personnel on shared system and performance improvement projects, as well as, provide oversight of the respective committees and survey preparedness and readiness.
Develops and establishes operational and strategic goals and objectives for the Chest Pain and Stroke Programs consistent with current guidelines from the American College of Cardiology, American Heart Association, and The Joint Commission for Accreditation, including Performance Improvement and a Patient Safety program.
Coordinates the development, implementation, review, and revision of Program standards, policies, procedures, and protocols.
Incorporates knowledge of ACC, AHA Guidelines as well as emerging science and evidence based practice into program standards.
Leads and participates in program performance improvement processes.
Assists in data gathering, medical record extraction for analysis, trending and review.
Monitors the Operational Efficiency of the Programs.
Promotes marketing efforts for the Programs to internal and external audiences that will support the Program image to the community.
Provides collaborative communication to the department director and leadership team
Develop and deliver continuing education programs. Plans, coordinates and evaluates education for nursing and providers related to the care and management of a stroke and chest pain patient in collaboration with Program Medical Directors and the Education department.
Provides related education to internal and external audiences, including planning and participating in educational programs targeting hospital personnel, EMS providers in the local and regional communities.
Participate in educational opportunities appropriate to professional goals.
Remains current in research related to the stroke patient populations and stroke systems of care.
Interprets and applies current research into clinical practice.
Participates in research, as appropriate
Participates in the development, implementation, and analysis of research projects and communicate findings through presentations and publications.
Chairs relevant Program Committees related to quality improvement and regulatory adherence.
Actively participates on internal, local, regional, and national advisory groups to work to advance the science of care for the person experiencing stroke/chest pain
Supports delivery of evidence-based acute stroke assessment and management.
Provides consultation and practice oversight.
Rounds on Patients prior to discharge
Evaluates patient outcomes and identifies and intervenes on variances.

Shares lessons learned from Post STEMI debrief sessions.
Support the process for the transitions of complex patients between multiple specialty and primary care providers and between healthcare settings and home.
Professional Requirements:
Reports to work on time and as scheduled, completes work within designated time.
Adheres to scheduling requests of the department (Works weekends, no “special days off”, no exceptions).
Adheres to the Principles of Service of Baylor Scott & White Medical Center - Sunnyvale while representing the organization in a positive and professional manner.
Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department and complies with all organizational policies regarding ethical business practices.
Utilizes computerized punch time system correctly.
Completes annual education requirements and maintains regulatory requirements.
Adheres to dress code, appearance is neat and clean and wears identification while on duty.
Completes in-services and returns in a timely fashion and has not missed any mandatory in-services.
Attends staff meetings when offered, is responsible for information presented at monthly staff meetings that he/she is unable to attend.
Actively participates in performance improvement and continuous quality improvement (CQI) activities.
Regulatory Requirements:
Required:

Graduate of an accredited School of Nursing
Preferred:

Academic degree in nursing (Bachelors or Master’s degree)
Current State of Texas Registered Nurse license.
Current AHA BLS Certification.
Current AHA ACLS Certification
Language Skills:
Ability to read and communicate effectively in English.
Additional languages preferred.
Skills:
Basic computer knowledge.
Knowledge of medical terminology.
