About the Role:



  • Serve as a key member of the Structural Heart team, coordinating seamless care from referral through discharge and long-term follow-up

  • Collaborate closely with cardiologists, cardiovascular surgeons, nurse practitioners, Electrophysiologists and multidisciplinary teams

  • Support pre-, peri-, and post-procedure care for Structural Heart patients, including TAVR and Watchman/LAAO procedures 

  • Act as a liaison between physicians, patients, families, nursing staff, and referring facilities to ensure continuity and quality of care


How You’ll Drive Our Mission Forward:



  • Assess patient conditions using critical thinking and clinical judgment; implement and evaluate individualized nursing interventions

  • Coordinate patient care across the continuum, including pre-admission testing, inpatient stay, 30-day and one-year follow-ups

  • Provide comprehensive patient and family education, ensuring treatment plans are understandable, feasible, and supported by available resources

  • Assist with case scheduling, medical record review, telephone triage, follow-up calls, and case review presentations

  • Ensure compliance with Medicare National Coverage Determinations for TAVR and other structural heart procedures

  • Contribute to quality improvement initiatives, protocol development, registry data collection (TVT), and outreach efforts with referring providers


What Makes You a Great Match:



  • Graduate of an accredited school of nursing with current Louisiana RN licensure

  • Minimum of 2 years of nursing experience, preferably in cardiology or cardiovascular care

  • Current BLS and ACLS certifications

  • Strong organizational, communication, and care-coordination skills with a patient-centered focus

  • Ability to collaborate across disciplines while demonstrating integrity, compassion, dedication, and commitment to safety and confidentiality
