Job Description - Student Assistance Program Liaison

The SAP (Student Assistance Program) liaison will assist in the identification of non-academic barriers to learning and in collaboration with families refer to community services that will enhance their school success.   This position has the responsibility of interfacing with identified school districts in Allegheny County, in order to identify and link behavioral health and drug and alcohol services for referred students.   This individual will provide screenings for youth identified through the SAP process and will provide advocacy, assessment, outreach, linkage, and assist with the coordination of resources and services for the identified client’s and families.   Work is performed with considerable degree of initiative and independent judgment in accordance with established regulations, policies and procedures.   The position reports directly to the supervisor as well as consultation with the unit manager and Executive Director.


DUTIES AND RESPONSIBILITIES
 
  1. Input billable units monthly reflecting daily activities in the WITS system, non-billable activities log and identified documentation in EVOLVE
  2. Complete initial SAP required trainings as identified by the state and county
  3. Completion of  CANS re-certification (as identified by the county)
D.  Attend SAP core team meetings at assigned schools
E.  Complete SAP CANS assessment with referred children within 2 weeks from the date of receiving parent consent to screen
  1. Communicate with and assist parents in the acquisition of identified services and resources.
  2. Report  updates in SAP core team members regarding recommendations and findings
  3. Participate in professional development and in-service for teachers and administrators
  4. Facilitate and assist with postvention efforts as requested
  5. Facilitate student psycho-education groups in schools as identified by the district.
  6. Provide follow-up with parents and students
  7. Participate in team maintenance and program evaluation activities with core team members
  8. Facilitate and support the school based aftercare plans for students who are returning to school from placement/ treatment
  9. Complies with and adheres to HIPAA confidentiality policy.
  10. Coordinate with families, providers, Children Youth and Families, juvenile probation, educational system, ODS and any other community resources. 
  11. Attends and participates in personal development and remains competent and knowledgeable of current trends, initiatives, resources and services available for clients and families.
  12. Complete additional 12 hours of approved supplemental training yearly.
  13. Participate in other HSAO programs during non- school months.
  14. Represent the agency at SAP or school related meetings and committees
  15. Other duties as assigned.

SKILLS AND KNOWLEDGE
 
  1. Ability to work in a team-oriented atmosphere.
  2. Ability to work collaboratively with other personnel, schools or service providers.
  3. Ability to be attentive to detail.
  4. Demonstrates strong organizational skills.
  5. Ability to adapt and adhere to all defined SAP timelines.
  6. Ability to be a self-starter and creative in identifying solutions to problems; knowing when to inform supervisor when obstacles arise.
  7. Knowledge of Microsoft Office products including Word, Excel, and PowerPoint.
  8. Ability to adapt to and learn new software products.
  9. Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities.

ESSENTIAL REQUIREMENTS
 
  1. Possess Act 33, Act 34 and FBI clearances
  2. Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
  3. Have current auto insurance that meets minimum collision coverage
  4. Completion and mastery of all required SAP trainings
  5. Strong oral and written communication skills.
  6. Strong interpersonal skills and ability to present to groups
  7. Ability to work independently.
  8. Full-time position requiring 40  hours per week


EDUCATION AND EXPERIENCE

Master’s Degree OR Bachelor’s Degree with one-year experience in the behavioral health or educational system.
