Shift: Full- Time
Schedule: Days, flexibility required


Seneca Health Care Center is looking to temporarily add a Subacute Coordinator- LPN to its nursing leadership team. In this role, you'll oversee the daily operations of an assigned nursing unit, ensuring high-quality resident care, staff supervision, regulatory compliance, and exceptional customer service. RESPONSIBILITIES:



  • Direct, coordinate, and supervise the activities of nursing staff on an assigned unit

  • Assume responsibility and accountability for the overall management of resident care within the unit

  • Monitor resident conditions and ensure care plans are developed, maintained, and updated as needed

  • Collaborate with physicians, interdisciplinary team members, residents, and families to support quality care outcomes

  • Communicate staff concerns and operational issues to appropriate department leaders

  • Ensure compliance with facility policies, state and federal regulations, and nursing standards of practice

  • Support staff development, coaching, performance management, and team engagement

  • Assist with admissions, discharges, family communication, and resident care coordination

  • Promote a safe, positive, and resident-centered environment

  • Perform additional duties as assigned


REQUIREMENTS:



  • Current and valid New York State LPN License required

  • Minimum of one year of experience in nursing service administration required

  • Additional experience in rehabilitative or geriatric nursing preferred

  • Strong knowledge of state, federal, and local long-term care regulations

  • Excellent leadership, communication, and organizational skills

  • Strong customer service skills and a compassionate approach to resident care

  • Must be flexible, dependable, professional, and able to manage multiple priorities
