We’re looking for a Substance Use Disorder (SUD) Care Manager to join our team at the Pittsburgh Mercy Family Health Center (PMFHC). In this role, you’ll work closely with both internal teams and external providers to deliver integrated, patient-centered care.
As the SUD Care Manager, you will provide clinically appropriate case management to individuals receiving services at PMFHC, with a focus on supporting their recovery and encouraging active participation in their care plans. You’ll play a vital role in promoting independence while maintaining therapeutic boundaries, and you'll serve as a key member of our multi-disciplinary care team.
This position also includes leading and managing our Medication-Assisted Treatment (MAT) clinic, ensuring the smooth operation and coordination of care for individuals with substance use disorders. You’ll support all relevant aspects of each patient’s care plan as needed to ensure continuity and quality of care.
SHIFT: (non-exempt/hourly)
Monday through Friday 8a-4p, with one 12p-8p shift per week
REQUIREMENTS:
Master’s Degree in Human Services + 1 year of experience OR Bachelor’s degree in Human Services +3 years experience.
Experience and/or interest in working with the homeless, at risk and individuals with substance use disorder and/or behavioral health diagnoses.
