- Supports the development and implementation of high-quality programs and resources for local youth and families.
- Engages in advocacy of the relevant supports and other needs of individuals and the community residents by bridging the knowledge, relational and service gaps between target communities and the organization.
1. Staff family’s case with the Intake Specialist once assigned and make contact with families to conduct Care Coordination sessions.
2. Prepare and assess information for a Family Team Conference (FTC). Complete a Family Team Conference, finalize Success Plan and link to services.
3. Conduct regularly scheduled assessments and Post Assessments to ensure a smooth termination and/or transition process, when necessary.
4. Meet directly with Clients and Families to provide support in completing their success plans and accompanying them to appointments, when needed.
5. Promotes health and wellbeing within a community through implementing wholesome programs and support
6. Maintain updated family records with plans, notes, appropriate forms, or related information.
7. Identify or contact members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or pregnant women.
8. Design and implement effective follow up systems e.g. contact clients in person, by phone, or in writing to ensure they have completed required or recommended actions.
9. Promote programs by distributing flyers, brochures, or other informational or educational documents to inform members of a targeted community.
10. Attend community meetings or health fairs to understand community issues or build relationships with community members.
11. Advise clients or community groups or peer support groups to support social connections.
12. Collect information from individuals to compile vital statistics about the general health of community members.
13. Advise clients or community groups on issues related to social or intellectual development, such as education, childcare, or problem solving.
Relevant Expereince
- At least two years of relevant experience working with similar family and youth populations;
- Counseling or advocacy related experience is an added advantage.

Affirming YOUth Foundation, Inc., (formerly Jonathan Spikes Foundation, Inc.), founded in 2011, is a not-for-profit community-based organization established to provide trauma-informed supportive services emphasizing prevention, harm reduction, mental health education, screening for trauma exposure and related symptoms, and evidenced-based, culturally relevant programs to youth and families in Liberty City, Miami, Florida.
We combine community expertise, local knowledge, experience, resources, school district leadership, university human capital, and strategic service providers positioned to reduce the incidence of violence through evidenced-based services to trauma-impacted youth. The objectives of our interventions are to improve youth social and emotional wellness, increase student’s life and coping skills, improve academic performance, reduce the occurrence of violence, improve school and community climate based on participants, and improve family engagement.
The goals of our interventions are to reduce the incidence of violence through health and wellness services to trauma-impacted youth; lower rates of aggression and violent behavior by assisting youth to develop emotional self-awareness and control, positive social skills, problem solving, conflict resolution, and aspiration for positive academic and social outcomes; provide intensive wraparound services that address environmental factors, family and daily living; and implement a community-based platform to improve positive youth and family outcomes while preventing violence, delinquency, and victimization through a continuum of care.
