SUD - SRH Case Manager

Position Title:  Substance Use Disorder / SRH Case Manager

Position Summary: Clinica Romero’s Substance Use Disorder Case Manager /SRH Case Manger will provide Case Management to provide SRH Education, to patients either during intake or during treatment. SRH Case Manager will assist clients in meeting program goals and objectives, document all progress and facilitate group sessions as needed. Assist with the outreach efforts and promote Drug and Alcohol Department Services to the community.

Responsibilities:

• Provides SRH Education Services.


• Provide Client Screening, Appointment Scheduling, Reminders & Follow-Up, Service Navigation, Referral & linkage to a Sexual and Reproductive Health specialist.


• Provide assessments, interventions, individual sessions, treatment plans, urine analysis screenings, intakes, case management, crisis intervention, level of care changes, justifications, and discharge summaries.


• Work with SUD counselors and LPHA’s to integrate SRH services to PPW population in treatment to ensure they are integrating SRH needs with their substance use treatment needs.


• SRH Specialist conducts Outreach & Engagement to insure all patients are getting informative information.
  


• SRH Education Services Coordinate all SRH services for their agency related to each PPW participant’s needs, including but not limited to: ensuring that agency staff retrained in assessing SRH patient needs.


• Establish relevant referral networks for family planning, prenatal and postpartum care; and coordinating with the SAPC SRH Services Manager for project updates by attending SRH Specialist meetings.


• Attend all meetings concerning SRH which are set by but not limited to LA County, SAPC, DHCS, DHS county/state/external and Internal participants entities in person or on line.


• Attend meetings, community events and outreach to insure we are meeting capacity for specialty services.


• Document all services provided within 48 hours of service date in SAGE/ Electronics Health Records and paper chart.


• Ensure all patient charts are meeting county and state requirements; all conversations, referral, phone calls, all case managements documented and billed for.


• Must meet billing numbers daily set by Program Director.


• The SRH Case Management is responsible for billing outcomes provided by management.


• Responsible for completing minimum of 35 billable hours per week Submits billable hours and calendar on a weekly basis to Program Director/Quality Assurance.


• Assist with Invoices for billing.


• Meet daily outcomes related to grant.


• Initiate discussions with all patients about their SRH needs, provide patients with appropriate referrals depending on their individual needs, follow up with patients to remind them of their upcoming appointments and coordinate with them to provide transportation, if needed, and provide referrals to ancillary services and brief intervention services when indicated.


• Assist with invoicing and insuring Linkages Act as a resource for staff when making referrals to external services (DOCUMENTATION AND REPORT billed, completed in a timely matter. 


• The SRH Case Manager must make sure all staff at their agency complete and submit the Reproductive Health data form and any other required elements in the electronic health record Sage in a timeframe determined by DPH-SAPC, ensuring that all data elements, which include, but are not limited to demographics, screening, referrals, education sessions, and engagement, are complete and accurate. Services each week for providing the SRH activities described.
  


• Responsible for recruitment of new clients in both the youth and adult population


• Case Manage/Counsel both youths and adults that have substance abuse problems and SRH.


• Transport clients to and from program if needed.


• Other assigned duties.
  

Qualifications

• Must be registered through CCAPP, CAADE, or qualifying certified body


• A minimum of one year’ experience working in the field of addiction.


• Must have completion forty-five hours of formal education in SUD and working in completion.


• Must be highly motivated and demonstrate good communication skills.


• Must be able to deliver strong, high-energy outreach to the community.


• Must have the ability to recruit new clients (adults and youth) to our agency.


• Must have an understanding of the 12 Core Functions.


• Must have the ability to write individual and group progress notes/treatment plans that meet county standard.


• Must have experience in observed drug testing of clients.


• Spanish bilingual is plus.


• Proficient in Microsoft Office.


• Experience working with Drug Medi-Cal clients (Adult and Youth) a plus.


• Understanding of Drug Medi-Cal requirements/process a plus.


• Must have knowledge of SAGETop of Form.
