Supervises FTEs: Yes Number of FTEs Supervised: 5+ Exercises full management authority including performance reviews, discipline, termination and personnel hiring Yes Annual Revenue in Dollars: N/A Annual Expense in Dollars: N/A Population Served Age of Population Served - Choose all that apply ☐ Infant (birth - less than 1 year) ☐ Pediatric/Adolescent (1 - less than 18 years) ☒ Adult (18 - 64 years) ☒ Geriatric (65 years & older) ☐ All Age Groups (Birth & Above) ☐ Not Applicable Organizational Standards § Provides direct supervision of Independent Care Services / Safety Sitter staff including: o Onsite supervision o Annual reviews o Management of productivity o Monitoring of compliance with Fidelity and State regulations § Keep progress notes using the tools and processes established by Fidelity Health care, provide summaries and reevaluations if indicated, and submit documentation according to established policy. § Participate in multidisciplinary process improvement teams as appropriate. § Provide Independent Care Services representation and leadership for Fidelity Health Care in professional settings as appropriate. § Responsible for day to day operations of Independent Care Services. § Responsible for opening Independent Care cases and managing patient cases and payor sources. § Participates in marketing and further development of Independent Care Services. § Attend scheduled staff meetings. § Maintains professional growth and development. § Acts as a role model for customer service activities. This position requires daily interaction with Independent Care Services, administrative staff, physicians and consumers. He/she must demonstrate the ability to deal effectively with all internal and external customers. This may include travel to other designated locations. Type of degree: Associates Area of study or major: LPN or Business Management Preferred educational qualifications: Graduate of accredited LPN School or Associates Degree Position specific testing requirement: N/A Minimum Level of Experience Required: 1 - 3 years of job-related experience Minimum of one year supervisory experience, preferred One year experience in a health care setting (hospital, clinic, rehabilitation center, home care, etc.). Excellent customer service skills required Experience with office management skills: customer services, answering phones, filing, etc. Must be capable of standing, stooping, bending and lifting up to 50lbs LPN, STNA or CNA,
