The Supervisor of Complex care is responsible for supporting the day-to-day operations of complex care management across the larger system of SPHP, including training, coaching, development, and evaluation of employee performance combined with designing and operationalizing workflows around complex patient progression. The supervisor oversees and facilitates continuity of care and coordination of services of complex patients as defined or determined by the hospital senior leadership team. The Supervisor of complex care management will analyze available data to measure individual and team performance, clinical program performance, department processes, and compliance. The supervisor of complex care management will anticipate and resolve complex issues and ensure accurate and cost-effective operations. In addition to the day-to-day responsibilities, the supervisor of complex care management will work in conjunction with the Clinical Executive of SPHP’s Integrated Care Coordination system on a variety of critical tasks including the management of one or more focused clinical initiatives by using a high level of creative problem solving and advance communication skills while performing under mainly autonomous conditions. The supervisor would be able to identify barriers which impact optimal patient functioning, throughput through the care continuum and ensures patient receives appropriate resources in most appropriate setting. The supervisor of complex care serves as a subject area expert in regulatory/policy issues that may impact progression of care, i.e.: OMRDD, Surrogate Decision-Making Act, Guardianship, end of life care etc.  The supervisor of complex care functions as agent of change to by actively participating in Quality Improvement initiatives within the department, hospital, organization and/or community and identify process or systems improvements. The supervisor of complex care will both oversee as well as assist their team in transitional care planning for the most complex patients within the acute or ambulatory setting, across one acute.  The supervisor of complex care will also assist with daily Care Coordination team needs as needed.

Principal Responsibilities:

The Complex Care Manager can be a RN or Social Worker with experience. This role will be responsible for all aspects of the Job Description depending on their licensure, including, but not limited to, the following: coordinating continuum of care activities for identified patients and ensuring optimum utilization of resources, service delivery, and compliance with external review agencies’ requirements.  In addition, the role encompasses the following responsibilities:

  • Develop and maintain collaborative relationships with internal and external stakeholders
  • Responsible for the planning, supervision and coordination of Complex
  • Care services across One Acute and all sites in collaboration with the Senior Director of ICCS.
  • Continually evaluate and monitor full portfolio of Behavioral Health, Complex Care and Social
  • Services offerings to ensure ongoing program development and that patient needs are being met and in compliance with regulatory standards
  • Develops, analyzes, monitors, and presents data and reports as relevant to Clinical and Operational leaders
  • Works with managers to optimize staffing and workflows, to achieve targeted goals for patient throughput
  • Ensures departments are appropriately resourced for the efficient functioning of a Complex Care program
  • Management and supervision of assigned case management/social work acute care staff
  • Hires, oversees training and reviews performance of staff
  • Communicates to staff changes in policies and procedures
  • Serve as a resource for staff: new information, questions, challenging patients and situations, escalation path
  • Provides in-service/training in revisions of policies as needed
  • Responsible for recruitment, hiring, development and training of all Complex Care staff• Works collaboratively and effectively with directors and managers to ensure the goals, mission and vision of the organization
  • Functions as a liaison between the department employees, acute/primary care/system leaders
  • Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes to program direction as well as policies and procedures according to new/updated regulations.
  • Leads and facilitates process improvements in patient access measures, clinic flow, patient experience and population-based initiatives
  • • Develops and implements complex care rounds across one acute while also participating in complex care management initiatives across the greater system/community
  • Assumes responsibility for the complex care management needs not only across the medical inpatient beds and emergency room but also the behavioral health service line and their patients with complex needs,
  • This position will assume development of workflows that integrate complex care management across the entirety of the SPHP system to ensure seamless transition, mitigation of barriers and systemic thinking and problem solving to improve the lives of those we serve as well as reduce workplace violence.
  • This position will serve as a key stakeholder in the development and operations behind the BEST team huddle program as well.

Education Requirements:

  • RN with BSN; Master’s degree or Certification in Case Management preferred.  Extensive case management experience may be considered in lieu of advanced education.
  • SW with MSW or Certification in Case Management preferred. Extensive case management experience may be considered in lieu of advanced education.

Experience Requirements:

  • At least 5 years case management experience required.
  • Highly articulate with strong written and verbal communication skills. Demonstrated ability to act in an assertive, diplomatic and innovative manner. Demonstrated public presentation skills.
  • Demonstrated ability to maintain cooperative relationships with medical staff, nursing and other professional staff.
  • Demonstrated leadership ability.
  • Computer literacy and analytic skills required.
  • Demonstrated ability to work with all age groups of patients.
