Supervisor/Intake Discharge Coordinator

Description

 The Partial Hospitalization Program (PHP) Intake Discharge Coordinator manages the day-to-day operations of the Dutchess County PHP including staff recruitment, training, and supervision; referral management and intake; data collection and reporting; on-call coordination; case coverage; and necessary administrative tasks.
