Hours: Mon-Fri 10a-6p FLEX (40hr)

Starting Pay: $19.50/hr

Job Summary:

Pathfinder is seeking a Supported Living Manager-1 (SLM-1) to oversee services for individuals with intellectual and developmental disabilities in a single supported living location. This role includes supervising Direct Support Professionals (DSPs), managing staff schedules and medical appointments, and ensuring high-quality, person-centered care that aligns with each individual’s goals and support plan.

As an SLM-1, you’ll be the direct point of contact for up to four individuals, helping them live independently and engage with their communities. You’ll lead a team of DSPs, support personal and health needs, and ensure compliance with Medicaid Waiver and state regulations. You’ll also advocate for individuals in team meetings, assist with financial tasks, maintain documentation, and work closely with leadership to ensure the success of the home.

Key Responsibilities:

As an SLM-1 at Pathfinder, You Will:

Lead and Oversee Services

  • Schedule staff and medical appointments for individuals supported.
  • Supervise and train DSPs in providing consistent, person-centered support.
  • Coordinate daily living activities, transportation, and personal goals.
  • Complete and upload quarterly summaries and track PCISP goals in Provide.

Ensure Safety, Health, and Compliance

  • Manage medications, appointments, and health-related documentation.
  • Ensure all Individual Support Plans (ISPs) and Behavior Support Plans (BSPs) are followed.
  • Maintain the home’s safety, oversee maintenance, and plan for budgeted upgrades.

Support Teams and Communication

  • Act as the main point of contact for families, case managers, and IDT members.
  • Facilitate staff meetings and help DSPs build strong, respectful relationships with the people they support.
  • Participate in an on-call rotation to ensure shift coverage and emergency support.

Document and Report

  • Track and report on individual progress, behavior, activities, and finances.
  • Ensure Medicaid and BQIS documentation is completed accurately and on time.
  • Monitor and adjust funding and waiver budgets to meet service needs.

  • Ability to lift up to 50 lbs and perform approved physical interventions if needed
  • Flexibility to work variable hours, including participation in on-call rotation

Requirements

  • Preferred Minimum Qualifications:
  • Minimum of a high school diploma/GED.
  • Prefer 2 years experience in the field of Human Services.
  • At least 18 years of age.
  • Basic language, written, and computer skills necessary for communication and documentation.
  • Demonstrated ability to support people with disabilities.
  • Ability to work a flexible schedule if needed.
  • Crisis prevention, intervention, and teaching skills.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Ability to speak, hear, and read the English language.
  • Be able to run, squat, stoop/bend, kneel, climb stairs, lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed. He/she shall have adequate vision, writing, and documentation skills to perform the essential functions of the job.
