About the Role
Are you a problem-solver with a passion for advocacy? As a Targeted Case Manager, you won't just be managing a caseload—you’ll be the bridge between our participants and the life-changing resources they need. You will work on the front lines, helping individuals with mental illness, navigate healthcare, housing, and social systems to achieve independence and wellness.
What You’ll Do
• Assessment & Planning: Conduct comprehensive assessments of participants needs and develop individualized, person-centered service plans.
• Advocacy: Act as a fierce advocate for participant’s rights and access to state, federal, and local benefits.
• Coordination: Link clients with medical, social, and educational providers; monitor service delivery to ensure it meets the participant’s evolving goals.
• Crisis Intervention: Provide immediate support and resource linkage during critical situations.
• Documentation: Maintain high-quality, clinical documentation in compliance with Medicaid standards.
Who You Are
• Qualified: You may hold a Bachelor’s degree in Social Work, Psychology, Sociology, or a related human services field.
• Organized: You can juggle multiple priorities and deadlines without breaking a sweat.
• Empathetic: You possess a deep understanding of the challenges faced by vulnerable populations.
• Mobile: You have a valid driver’s license and reliable transportation for community-based visits.
