About the Program and Position:

At Cayuga Centers, we know how important mental wellness is for you and your loved ones. That's why we offer our Child and Family Counseling program, which aims to help individuals improve their emotional well-being and strengthen family relationships. Our program includes individual, group, and family counseling, as well as psychiatric services for children, youth, and adults who are dealing with a wide range of challenges like depression, anxiety, ADHD, trauma, and more. How you get to use your skills…

  • Provide the following in partnership with the family: strengths/needs/culture discovery assessment, crisis planning, plan development, assistance in accessing community services, review of needs and monitoring progress
  • Assist parents/guardians in improving their skills to optimize the home environment by enhancing support systems and increasing the family's ability to problem solve and advocate for themselves and their children
  • Responsible for ensuring all activities of the case management process; fromadmission to discharge
  • Coordinate team meetings with other providers, including clinicians, parent support partners, natural supports, etc
  • Facilitate integration across systems (ie Child Welfare, DJJ, Education)
  • Maintains current and accurate records. This shall include but not be limited to intake, assessment, plan of care, progress notes, referral logs
  • Provide supervisor with all assigned reports and update information in a timely manner
  • Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall service plan goals as well as ensure immediate changes are made in service plans as individual's needs change; educate and support clients' families; and advocate for clients' rights and preferences
  • Provide outreach services to the individual in their natural environment to foster and support the case management relationship and to ensure engagement in their service planning
  • Ensures the client's rights are safeguarded throughout the referral, placement, treatment, review and discharge processes (Chapter 394, Florida Statutes and Chapter 65E-10 FAC)
  • Document all activity in progress notes and complete all other documentation required by the Florida Administrative Code and Medicaid community mental health and targeted case management service guidelines to maintain a permanent record of client activity according to established methods and procedures

Schedule:

Looking for an applicant who is willing to work with the youth/families at a time that suits the youth and family needs on a flexible schedule. This is a full-time, 40-hour-a-week position that includes evening and weekend hours.

Physical Requirements:

  • Ability to drive and operate a motor vehicle with passengers for at least four consecutive hours
  • Ability to participate in physical activities with program youth, including running, jumping, bending, and twisting
  • Ability to sit or stand for prolonged periods
  • Ability to walk up and down stairs repeatedly


  • Must hold a Targeted Case Management Certification from the Florida Board
  • Must hold a bachelor's degree in behavioral science, human services, or related field
  • At least one year of experience with children, youth, and families
  • Communicate in and comprehend the English language effectively both orally and in writing.
  • Ability to form a mutually respectful and balanced relationship with the youth
  • Ability and willingness to engage and work with families in an "in-home" context
  • Knowledge of Child Welfare and Foster Care Systems
  • Ability to apply youth and family strengths to inform treatment planning
  • Flexibility to work when the families are available to meet, including evenings, weekends, holidays
  • Ability to carry out interventions creatively to support the client's goal achievement
  • Ability to function independently and willing and able to seek guidance as necessary
  • Valid FL Driver's License
  • Vehicle required: Must be safe and dependable as program youth are transported
  • Ability and willingness to drive to client's homes within a reasonable distance from the office, which may include neighboring counties
  • Excellent communication, organizational, and collaboration skills
  • A strong belief in the ability of people to grow and change
  • Possess the knowledge, skills, and attitude needed to appreciate and encourage diversity and understand the concept of cultural trauma as it applies to individual and family functioning
  • Bilingual: Spanish-Creole-French, required
