Job Description - Team Leader, Daily Supported Community Living

Description

The Team Leader for Daily Supported Community Living provides leadership and oversight for the daily operations of assigned client homes, ensuring the delivery of high-quality, person-centered support. This role involves managing a team of Direct Support Professionals, (DSPs), coordinating client care, and fostering a positive and empowering environment for individuals with disabilities to achieve their goals and live fulfilling lives.  

Principle Accountabilities:

  • Leadership and Team Management: 
    • Recruit, onboard, train, and develop Direct Support Professionals (DSPs).
    • Provide ongoing feedback, coaching, counseling, and performance management through observation and monthly meetings.
    • Facilitate team meetings to address operational components, review policies, and align with agency goals.
  • Client Care Coordination: 
    • Coordinate assigned client homes, ensuring effective communication with care teams.
    • Assist clients in developing and achieving personal goals, monitoring progress, and adjusting plans as needed.
    • Plan and coordinate client activities to meet individual needs and preferences.
    • Conduct and coordinate client annual staffing meetings.
    • Facilitate open communication with guardians.
  • Compliance and Documentation: 
    • Ensure client files comply with funder and agency standards.
    • Maintain accurate and timely progress notes and documentation.
    • Administer medication according to Medication Manager Certification and agency policy.
  • Program Support: 
    • Assist in recruiting and assessing potential clients.
    • Maintain a working knowledge of supported community living needs.
    • Work effectively with the SCL leadership team to staff homes, including providing direct support as needed.
    • Attend and actively participate in Operations Team meetings.
    • Perform other duties as assigned.

Requirements

 Knowledge, Skills, and Experience Requirements:

  • Bachelor’s degree or four years of experience in human services, or equivalent experience preferred.
  • One year of experience providing direct care, preferably to individuals with disabilities in an independent living setting.
  • Strong leadership and support skills.
  • Excellent verbal and written communication skills.
  • Ability to interact positively and professionally with clients, team members, guardians, and stakeholders.
  • Ability to make independent decisions and handle emergencies.
  • Access to a telephone.
  • Driving record that meets agency standards for insurability.
  • Access to reliable transportation.
  • Valid License Class D (noncommercial – chauffeur) license.
  • Ability to obtain Medication Manager Certification within 90 days of employment.
  • Proficiency in Microsoft Office.
  • Ability to learn and utilize agency databases.
  • Ability to bend, stoop, twist, and lift.
  • Ability to lift a minimum of 80 pounds.
  • Ability to work evenings and weekends as required.
