**Schedule: Monday-Friday 8am-4:30pm**
**Position Responsibilities:**
- Work within team environment agency-wide and in partnership with MAT provider, nurse and therapist.
- Assist teams in identifying appropriate referrals to MAT team.
- Identify and share with staff community resources that foster improved health overall.
- Educate consumers, family, and natural supports of MAT, addiction and necessary or recommended interventions.
- Schedule transportation for consumer to attend appointments, as necessary.
- Support Consumers by initiating collateral contact with appropriate releases of information.
- Develop logs or tracking mechanisms needed to assist with medication management.
- Ensure appropriate assessments and Care Plans are completed to measure readiness for change.
- Coordinate care with both mental health and physical health care providers.
- Ensure appropriate documentation is available regarding conservatorship, power of attorney and/or advance directives.
- Assist in helping CM teams with understanding appropriate use of the MAT program.
- Provide Consumers with appropriate referrals for needed services.
- Provide leadership during team meetings, Consumer staffing and crisis scenarios.
- Initiate creative ways to increase engagement.
- Maintain daily MAT reports
**Other Responsibilities:**
* Responsible for attending all mandatory in-services, continuing education and annual health requirements
* Comply with all Corporate Compliance policies, practices and procedures
Report all unethical activities to supervisor, Corporate Compliance Office, Values Hotline number or Human Resource Director
* Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor, Safety Supervisor and/or OSHA Coordinator
* Will be compliant with the relevant regulations concerning the privacy and security of consumers protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
* Participate in proactive team efforts to achieve departmental and company goals
* Perform other duties as assigned
* Provide leadership to others through example and sharing of knowledge/skill
* Follow all policies and procedures set by the Agency
**Essential Skills and Experience:**
* Bachelors Degree in health-related field of counseling, psychology, social work or other behavioral health field.
* Candidates with a Bachelors degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.
**Essential Requirements for the Position:**
