Eliot is seeking an enthusiastic Team Leader to join our Program for Assertive Community Treatment (PACT) team. PACT is an evidence-based model that provides comprehensive treatment, rehabilitation, and support to adults with severe and persistent mental illness.
The ideal candidate will work alongside the Team to ensure that recovery services, vocational services & clinical support are included in the treatment plan developed for each individual served.

Responsibilities:
- Oversee clinical, administrative and operational functions of the team while providing supervision to staff.
- Assist in the development and oversight of Treatment Plans, ensuring coordination of case management services including income, insurance, and appointment scheduling
- Assist consumers in being fully integrated in their communities by supporting them in the use of community resources, encouraging them in establishing memberships within their communities, and reducing stigma within the community through education.
- Ensure the safety of clients; implementing crisis intervention as necessary. Includes but not limited to: use of deescalating modes of communication/ interaction to ensure consumer’s safety, stability, mental status, and physical health
- Provide emergency twenty-four hour on-call back-up assistance to staff members who are on-call to clients.
- Ensure that staff provides side by side interventions to clients that focus on problem solving, skills training, modeling behavior, optimism and encouragement.

Qualifications:
- Requires a Master’s degree in Social Work or related field
- Licensure in Social Work or Counseling by the Commonwealth of Massachusetts required
- At least 3 years of experience working with adults with severe mental illness
- At least 2 years supervisory experience
- Valid Massachusetts Drivers License

Annual Salary
$95,000 - $105,000 USD
