The physical therapist is responsible for providing physical therapy services to clients of the agency. The primary function is the provision of direct patient care, including collaboration with other team members and participation in the development of the patient s comprehensive plan of care.

• Rehabilitation equipment including but not limited to:
• splints,
• orthoses
• prostheses,
• wheelchairs,
• ADL equipment
• Modalities

Traveler will float to BMC as/if needed

Assignment Location: 71 Hospital Ave, North Adams, MA 01247

Non-billable orientation: 8hrs
Holiday, OT Multipliers: 1.25%
Call, Call-Back and Charge: Not applicable for this need
Requirement description :

• 2 Years experience
• Bachelor (or higher) degree
• Valid Driver License and Vehicle required
• TRAVELER MUST BE WILLING TO FLOAT TO BERKSHIRE MEDICAL CENTER WHEN/AS NEEDED (Facilities are approx 21 miles apart)
• AT LEAST TWO SPECIFIC INTERVIEW DATES/TIMES OF AVAILABILITY FOR INTERVIEW MUST BE ADDED TO PROFILE - PROFILES WILL NOT BE PRESENTED WITHOUT THIS INFORMATION. "ANYTIME" WILL NOT BE ACCEPTED.

• Work History
• Skills Checklist (pertinent to position), current to 1 year
• 1 Supervisory Signed Reference (from within the past two years of employment)
• License/Certifications - Must be uploaded with profile (Current license, BLS, and Covid Vaccination proofs (dated on or after 10/3/23) or intent to file exemption form - Facility will not pend license/certs)

CERTIFICATION REQUIREMENTS :

• BLS (AHA)

STATE LICENSE REQUIREMENTS :

• Massachusetts

ADDITIONAL LICENSE REQUIREMENTS : MA License must be in hand at profile upload
Weekend Requirements : EVERY WEEKEND/Part of shift
