Job Description - Therapist Assertive Community Treatment (ACT)


Purpose:

The ACT Therapist provides community-based individual therapy, clinical, substance use disorder and support services to consumers and/or their support system utilizing the ACT Model. The ACT therapist will join the ACT multi-disciplinary team to serve those individuals with serious and persistent mental illness and presenting with the highest level of clinical intensity and at risk for arrest and/or higher levels of care. 

Accountabilities:

  • Provide community based intensive therapy community-based services to adult consumers with a severe and persistent mental illness and may have a co-occurring substance use disorder who have frequent contact with criminal justice entities. These may also include other community-based services such as case management as needed
  • Provides mental health, dual diagnosis and/or substance use disorder therapy, in both an individual and group setting, to consumers and families as appropriate
  • Will maintain caseload of client/consumers from the ACT team and other teams as needed
  • Assists with the development of diversion plans to help reduce/prevent hospitalizations and incarcerations; actively participate in team planning for implementation of diversion plans.
  • Attend daily multi-disciplinary team meetings for coordination of care for clients.
  • Actively participates in the research, development and implementation of emerging multi-system models and practices.
  • Coordinates community / mobile emergency responses in conjunction with the Emergency & Stabilization Services team as needed. 
  • Participate in on-call or after-hours responsibilities when required by the program.
  • Models inspire flexibility and creativity in the delivery of clinical services.
  • Outreach individuals who are in need of mental health services and have had frequent law enforcement contact.
  • Provide transportation and accompany clients to and from activities and/or appointments to provide medically necessary direct care and support, as directed.
  • Complete all required documentation thoroughly, on time and according to agency standards. 
  • Attend team meetings and individual supervision sessions as required by the direct supervisor.
  • Provides crisis prevention and intervention as needed.
  • Perform other duties as assigned.

Requirements

  Qualifications:

  • Master’s Degree and current licensure in Kansas as a QMHP and Master Level Addictions Counselor (LMAC). If not currently licensed as a LMAC, must be able to obtain licensure within 90 days of employment
  • Previous experience in mental health, homelessness, or crisis services preferred.
  • Ability to establish relationships with individuals and organizations of influence within the social services community, including civic groups, charitable agencies, related government entities, fundraising sources, etc.
  • Demonstrated interpersonal and communication skills.
  • Proficient in use of technology, including Microsoft Office.
  • Provide, maintain dependable transportation, current driver’s license, and auto insurance.
  • Ability to successfully complete required background screenings, including criminal history and Kansas Child Abuse/Neglect Registry clearance
