Description

We know there is someone out there that will make an excellent addition to our Behavioral Health team. Someone who gets along well with others, is thorough and efficient, can follow company procedures, is able to multi-task and has amazing customer service. Our clinic is a dynamic place to work, practice, and grow. We have 4 primary care health centers s and deliver integrated services including behavioral health, Pharmacy, Dental, specialty referrals, chronic disease management, health education, and much, much more. UNHS is hiring one (1) Full-Time (FT) position for a Therapist.
  • Location: All Locations (Montezuma Creek, Monument Valley, Blanding Family Practice, Navajo Mountain)

Critical Tasks

  • Provide clinical services to youth and young adults who may be suffering with a serious mental illness or serious emotional illness. 
  • Provide psycho-therapy services to youth and young adults.
  • Collects data using observation, interviews, and counseling sessions to specifically diagnose and assess behaviors
  • Works in conjunction with other behavioral health professionals such as Nurse Practitioners and Psychiatrists to derive an appropriate treatment plan
  • Provides interventions and therapy by identifying behaviors and prescribing effective activities for behavior modification
  • Provide crisis counseling services for individuals, and families as in-person or telehealth service and determine resources and referrals for higher level of care needs.
  • Works with other practitioners such as Nurse Practitioners and Psychiatrists to develop treatment plans and interventions for SMI (serious mental illness) clients with borderline personality disorders, suicidal thoughts, and self-harming behaviors.
  • Works closely with Navajo Traditional Counselors to provide and support culturally competent services
  • Collaborate with public health or other departments for outreach or educational services in the community. 
  • Provide crisis intervention, suicide intervention and safety planning, responsible for assessment, and determining level of care for patients.
  • Provide follow-up to high-risk callers reporting suicide and/or homicidal thoughts and behaviors.
  • Provide crisis intervention, suicide prevention, information & referral, and brief supportive counseling to patients who are in emotional distress and/or seeking information on available mental health services.
  • Utilizes group and individual therapy to engage clients in reconstructing non-productive behaviors and acquiring behavioral skills that will remove harm and diminish negative psycho-social behavior.
  • Attend and participate in Clinical Supervision and Behavioral Health Department Meetings as required by Program Director.
  • Uses a variety of skills such as counseling, communication, clinical, problem-solving, and interpersonal skills to monitor and manage prescribed treatment plans. Including Evidence Based Practices(EBP).
  • Maintains detailed client/clinical records in the EHR System and updates as required to assure behavioral modification goals are achieved
  • Tracks medications recommended by Nurse Practitioners and Psychiatrists to assess the impact upon client behaviors and determine the need to alter or change treatment plans reported in progress notes.
  • Instruct clients on how to use the necessary skills and exercises to build social, transitional, language, cognitive, and interactive skill development need for behavioral change
  • Hypothesizes situations and conducts situational analyses to assist clients with problem-solving activities needed for behavioral change
  • Counsels and prescribes interventions and works with other practitioners such as Nurse Practitioners and Psychiatrists to develop treatment plans and interventions for SMI/SEI (serious mental illness/serious emotional illness) clients with borderline personality disorders, suicidal thoughts and self-harming behaviors.
  • Utilizes group and individual therapy to engage clients in reconstructing non-productive behaviors and acquiring behavioral skills that will remove harm and diminish negative psycho-social behavior
  • Participate in Quarterly Chart Audits.

Requirements

  • Inputs client data in the EHR (electronic health records) or current record-keeping system
  • Collaborates with other licensed practitioners effectively to finalize, change or develop new clinical interventions
  • Collaborates with assigned Case Manager to align goals with treatment plans
  • Monitors client intake of prescribed medications and reports any noticeable behavioral changes to Prescribers
  • Communicates effectively with clients, client families, clinical staff, and other prescribers involved in the overall treatment plan
  • Utilizes current behavioral health techniques in cognitive therapy work with youth and young adults
  • Demonstrates compassion in communications, active listening, patience and interpersonal skills
  • Demonstrates sensitivity and an appreciation of the culture of the Navajo People
  • Shows adherence to policy, procedures of the Utah Navajo Health Clinics and understands the regulatory laws governing behavioral health practitioners in the State of Utah
  • Co-lead the weekly clinical staffing with case managers and peer support.
  • Provides service at any UNHS location as determined by the Program Director.

Metrics

  • Maintains appropriate licensure; licensed counselor or licensed social worker
  • Maintains ongoing CEU’s (continuing education credits) as required
  • Meets caseload requirements as prescribed by the agency
  • Provides appropriate reporting as required by the Chief Medical Officer for UNHS
  • Meets all auditing requirements for regulatory oversight

Qualifications

  • Master’s degree in Social Work or related field
  • Licensed practitioner in the State of Utah
  • Navajo preference
  • Prefer experience working with Native American communities

In accordance with Navajo Nation and federal law, UNHS has implemented an Affirmative Actin Plan pursuant to the Navajo Preference in Employment Act. The NPEA has the following preferences: 

1) Enrolled Navajo (must have a CIB). 

2) Applicant married to an enrolled Navajo and who has lived on the NN for more than a year. 

3) Everyone else (all other tribes and non-Indians). 

As a Tribal Organization the Utah Navajo Health System, Inc. (UNHS) treats patients with high risk and underlying chronic medical conditions. Therefore, UNHS requires its employees to show proof of immunization prior to their employment with UNHS or during their employment. Below is a list of immunizations/vaccinations required to be employed or to continue employment with UNHS.
The immunizations/vaccinations include but not limited to following:

  • MMR (Measles Rubeola, Mumps, Rubella): Documentation of two MMR vaccines (OR) Documentation of MMR titers.
  • Complete Hepatitis B vaccine series (3 doses), documented proof of titers indicating immunity, or a declination that may be signed upon arrival.
  • Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
  • Tetanus, diphtheria, and pertussis (Tdap).
  • Varicella titers, history of varicella or varicella immunization series.
  • Influenza vaccine for the current year.
  • COVID-19 vaccine
  • Any other vaccinations as requested.
