Summary:
Provides individual, family and/or group therapy as well as case management services to youth and families.Responsibilities:
Provides case management services, per client's need, after program completion.Coordinates clients' transportation for special activities and/or events.Per grant requirement, conducts intakes and assessments utilizing evidenced based curriculum techniques to provide individual, family and/or group therapy.Per grant requirement, completes all pre/post data collection, daily, weekly and monthly progress notes, treatment plans, treatment plan reviews, progress updates and program completion plan for each client (on case load).Reports clients' progress and outcomes to referral sources.Competencies:
