Job Description - Thp Case Manager Plus


The case manager will be responsible for working with up to 12 non minor (NMD) dependents and/or a ratio of up to 10 pregnant or parenting NMD. Responsible for coordinating with significant others, placing agency, and adjunct treatment members to enable the NMD to attain his or her goals. The case manager will not be responsible for providing after hour assistance to clients or on-call services. 

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

  1. Be knowledgeable of transitional housing programs and resources for transitional aged foster youth.
  2. Be knowledgeable in the Community Care licensing regulations for THPP for NMD
  3. Conduct a pre-placement appraisal with the placing agency and NMD. Explain the program expectations and complete necessary intake paperwork.
  4. Following acceptance into the program the social worker will obtain intake information and complete a needs and services plan. 
  5. THPP NMD social worker will complete initial an update the needs and services plan/ STLP and submit to the supervisor within 10 working days.
  6. Provide at least weekly consultation to the NMD. Consultation may but is not limited to discussion of progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, housing advocacy and tenancy training, computer training, transportation assistance, assist in contact with appropriate supportive adults/mentors.
  7. To provide assistance with managing the NMD’s cash resources, personal property, and valuables
  8. To provide assistance with maintaining accurate records of the NMD’s bank account, savings, and monthly budget.
  9.  To provide and/or coordinate referrals for additional services, consultants and community resources as needed (i.e. medical, therapy, psychiatric, parenting classes, job training,)
  10. Document the NMD weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts. 
  11. Complete special incident reports and any other reports as required by the placing agency. 
  12. Maintain contact with placement worker and inform of any significant events/incidents
  13. Consult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed.
  14. Meet with supervisor for a minimum of one hour weekly of individual supervision and case consultation.
  15. Ensure that client files are complete and up to date.
  16. Submit all reports and documentation to supervisor for final approval.
  17. Facilitate trainings, and support groups as directed by supervisor.
  18. At all times project a favorable image of Walden Family Services.
  19. Performs other duties as assigned or requested.

  • Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Requirements

QUALIFICATIONS REQUIREMENTS:

  1. Candidate must have a minimum of a bachelor’s degree in social work or a closely related field, as defined in the State Department of Social Services, Community Care Licensing regulations with at least 1-year prior experience working with the population 
  2. Must possess valid California Driver’s License, a working vehicle and have a good driving record.
  3. Fingerprint and Child Abuse Index clearances from the Department of Justice.
  4. Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility.

EMPLOYEE STATUS: non-exempt 

SALARY RANGE: Beginning salary will be commensurate with education and experience.
