Position Summary


The Thriving Texas Families Non-Medical basic needs coordinator will implement the core services of the program to the individuals who are eligible for services. This will include pregnant mothers, spouses, legal guardians, or foster parents who are pregnant or has a child between the age of 0- 3. Individuals identified in need of crisis support, referrals, case management, or mentorship. The goal of the program is to improve healthy pregnancies and deliveries; child health and development; and improve family dynamics. This position will provide directs services as to eligible individuals of the Thriving Texas Families program. This position will report to the lead community care coordinator and program manager. Position will coordinate with TTF eligible clients with the allowable non-medical basic needs support. This includes assessing the needed support, coordination of ordering and delivering approved basic support supplies. Other duties will also include program outreach, Services, and education are being completed to program design standards.



Minimum Requirements


Requires minimum of bachelors degree in behavioral health field of study; or 2 years case management experience in behavioral health field; or community health worker certified
Knowledge of available prenatal care systems and postpartum services
Ability to conduct screenings, assessments, service plans, and follow-up with identified patients
Requires a valid Texas drivers license, appropriate driving record and auto liability insurance that meets the minimum required by state of Texas statue
Reliable transportation
Competent with computer skills, email, web browsing, internet, word and excel processing
Highly motivated and able to work with a variety of groups
Independent worker with initiative and team values
Mental and physical health sufficient to meet the demands of the position



Essential Functions


Position will represent the program at relevant meetings throughout the community, and be familiar with community resources in order to conduct referrals and meet the needs of the targeted groups.
Responsible for maintaining program supplies with emphasis on office supplies, program supplies for participants, submission of approvals, and coordination of picking up supplies.
Coordination of Non-Medical basic needs support of eligible clients. Including, assessing need, delivery of non-Medical basic needs support items in a timely manner, maintaining required documentation for services, and maintaining inventory for TTF basic needs support items.
Assisting program manager and lead community care coordinator in submission of monthly programmatic reports. Includes combining all program activities conducted by all staff. Reports must be in required program templates to meet required deadlines. Required reporting will include all demographic data, referrals, rates of participation, and follow ups to support annual goals of contract and monthly performance measures.
Be able to facilitate all program activities including community outreach, recruitment, intake, referrals, client classes, individual and group sessions, and follow - up according to the established policies and procedures to meet required outcome measures.
Ensure all outreach materials are organized, updated, and culturally appropriate
Will report directly to the lead community care coordinator and program manager to assess programmatic goals and objectives.
Will work closely with the patients in an effort to keep them engaged in the medical system during pregnancy and facilitate referral to social services and basic life needs.
Position will communicate with staff any concerns or issues needing immediate attention
Assists senior staff in developing new program components, and other related duties.
Position requires a flexible schedule and may require working irregular hours
Carries out other duties as assigned



Required Skills


Must be able to communicate effectively in large group settings and demonstrate independent judgment
Ability to understand the overall goal of the program and implement activities that meet objectives.
Must be able to keep up to date and factual documentation
Energetic, forward-thinking and creative individual with high ethical standards and an appropriate professional image
The ability to work independently and effectively under pressure
Proficient in MS office suite (Ex: Outlook, word, excel, etc.)
Demonstrated human relation and effective communication skills also required
Superior organizational and problem solving skills



Physical Requirements


Requires sitting, standing and walking for extensive periods of time. Requires working under stressful conditions or working irregular hours. Requires frequent exposure to communicable diseases, body fluids, toxic substances, medicinal preparations and other conditions common to a clinic environment.


The employee frequently is required to reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Ability to work with a moderate noise level in the work environment is required.


The occupant may, at times, lift and/or move objects up to 30 pounds. The noise level in the work environment is usually moderate.
