Job Description - Training Counselor-Rose Center

Description

Essential Responsibilities (include but not limited to):

1. Serve as an Interdisciplinary Team Member for members assigned by Program Director / Coordinators

2. Provide input regarding the member for Discovery, PCPs, Implementation Strategies, Behavior Plans, etc

3. Attend training on Implementation Strategies and Behavior Plans as instructed

4. Apply methods described in Implementation Strategies, Behavior Plans and other plans pertinent to the member

5. Implement Principles of Person Centered Thinking

6. Follow Settings Rule requirements at all times

7. Support members in accessing the community, including 31C objectives

8. Support members achieve independence in all activities

9. Conduct individual/group activities at Program Coordinator request

10. Conduct individual/group activities independently

11. Ensure members control their own schedule, participation in activities of their choice

12. Support members in assessing their own progress and the progress of others

13. Support members in completing their own paperwork such as monthly summaries

14. Support members in using Quantum

15. Support members in gaining/keeping employment

16. Support members in utilizing the community on a daily basis

17. Support program use of Adventures, Expeditions and Neighbors programming

18. Respond to medical and behavioral emergencies following appropriate Health and Safety, Safety Care, First Aid, CPR, event/CIRUS reporting and other procedures as necessary

19. Successful completion of all required training for certification as Direct Support Professional (DSP)

20. Valid driver’s license required

SUPERVISORY RESPONSIBILITIES

No supervisory responsibilities.

Requirements

QUALIFICATIONS

To perform this job successfully an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.

EDUCATION, SKILLS, and/or EXPERIENCE

· Six months or more experience working with the I/DD population

· College coursework in related fields (social work/psychology/sociology/special education/rehabilitation/human services etc)

· Registration on the Illinois Health Care Worker Registry as a DSP (Direct Service Professional)

· Hobbies and/or skills in the area of dance, arts & crafts, theater, gardening, sewing, etc. are desirable qualities in the ideal candidate

PHYSICAL DEMANDS:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. MUST pass the Safety Care course with physical techniques that include but are not limited to: stability holds, leg wraps, and floor drops.

While performing the duties of this job, physical requirements may include standing, walking, and sitting; seeing; hearing and listening; clear speech; dexterity and use of hands and fingers.

WORK ENVIRONMENT:

The work environment described here is representative of what an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually low to moderate. The work environment may include any or all of the following working alone, working with others, verbal contact with others, and face-to-face contact.
