Job Description - Transition Age Youth Coordinator


Transition Age Youth (TAY) Coordinator  

Department: Allegheny County Department of Human Services 

Division: Children, Youth & Families 

Location: 901 Pennsylvania Avenue, Pittsburgh, PA 15223 

Summary 

The TAY Coordinator is responsible for providing organizational direction and administrative support for all divisions under the CYF TAY unit. TAY divisions are inclusive of the Educational Liaisons, Youth Engagement and Learning Specialist, and Crossover.  This position requires an individual to be enthusiastic about the learning process, committed to working with diverse populations and able to engage transition age youth, system partners, agencies, and other TAY staff within DHS.   

This individual is responsible for moving TAY special projects and programs forward while assisting with administrative and clerical duties as well.  This individual should be independently driven and self-motivated.  This individual will have close partnerships with the CYF TAY unit, DHS TAY system partners, TAY providers and TAY community resources.   

Must be able to travel locally and out-of-county and be available to work a flexible schedule, which includes evening and weekend work. 

Essential Duties and Responsibilities 

  • Foster relationships with all DHS TAY departments, community partners and contract providers to promote implementation of appropriate TAY programs/service. 

  • Assist TAY Manager with gathering data and outcomes regarding service provision for ongoing analyzation. 

  • Complete relevant administrative and clerical task for CYF TAY unit divisions. 

  • Assist with coordinating CYF TAY events such as post-secondary tours, educational/vocational/training events, and any other relevant opportunities for youth and/or providers. 

  • Assist with organizing professional development opportunities, training activities & team building events for CYF TAY unit. 

  • Participate in DHS department meetings and other relevant meetings as needed. 

  • Assist TAY Manager with overseeing internal TAY service provision, which is inclusive but not limited to referral oversight, NYTD survey completion oversight, providing youth credit reports upon request, stipend/concrete goods/gift card request. 

  • Support TAY Manager with development and implementation of new, modified and/or restored practices/programs as required by law, regulatory authorities and/or DHS. 

  • Assists in developing and implementing innovative TAY services. 

  • Effectively utilize Independent Living dashboard, Synergy, KIDS Database and any other required systems to document encounters, service provision and any other relevant information/data. 

Additional Responsibilities 

From time to time the employee will be required to perform additional tasks and duties as required by the employer. 

Knowledge, Skills and Abilities 

  • Ability to understand the mission/mandates of the Child Welfare system and the goals of transition programs. 

  • Knowledge of federal, state and county regulations related to transition aged youth in Allegheny County.  

  • Understanding of transition-based needs (housing, health, education, etc.). 

  • Knowledge of rules, regulations, policies, and procedures that relate to the provision of social services to children and families. This includes a thorough knowledge of the casework process, the foster care system, and the courts. 

  • Excellent interpersonal skills, communication skills and organizational skills; ability to relate in a professional manner to youth, families, and professionals, as well as public speaking skills. 

  • Knowledge of local community organization and TAY specific supports.   

  • Ability to make clear and pertinent statements, both orally and in writing. 

  • Skills in engaging youth who may be unmotivated or resistant.  

  • Excellent group facilitation skills amongst a multidisciplinary team. 

  • Excellent conflict resolution, with the ability to find creative solutions to problems/issues. 

  • Innovative thinking in regard to addressing service gaps through program implementation.

Education/Experience Requirements 

  • Master’s degree in a related field, plus one year of professional experience working in a human services or related field, specifically in public or private social work with transition aged youth. 

-OR- 

  • Bachelor’s degree in a related field, plus three years of professional experience working in a human services or related field, specifically in public or private social work with transition aged youth. 

  • Certificates, Licenses, Registrations 

  • Act 33, 34 and FBI clearances. 

  • -AND- 

  • Valid driver's license and access to a reliable vehicle.
