Job Description - Transition Coordinator

Position Summary

The Transition Coordinator will play a crucial role in facilitating smooth transitions for people we support, ensure effective employee training and manage the transition of caseloads and homes. This position will serve as a liaison between landlords and Sunnyhill, Inc. ensuring all parties are well-informed and coordinated. The Transition Coordinator will have strong organizational skills, excellent communication abilities, and a passion for supporting the people we support and their teams through all transition processes.

  • Review the consumer referral database to identify and evaluate potential new people we support for the department.
  •  Maintain a robust system for tracking referrals and intakes.
  • Lead the intake process within the department ensuring comprehensive communication with supported individuals, families, guardians, support teams, service coordinator, and external service providers.
  • Act as the agency liaison during meet and greets and throughout the intake process.  
  • Ensure all intake processes and documentation are completed and maintained. 
  • Coordinate with various departments to ensure a seamless onboarding and transition experience.
  • Conduct monthly post-intake meetings with the team for one-year post-transition.
  • Direct all move-related activities within the department.
  • Identify, tour, and secure properties that align with the needs of the people we support. 
  • Ensure compliance with documentation for rental property leases.
  • Serve as the liaison between Sunnyhill and landlords, ensuring all services outlined in the lease are effectively set up and completed.
  • Ensure the compliance of files for supported individuals.
  • Collaborate with internal departments to arrange training specific for the new home.
  • Perform environmental and ongoing reviews and site checks. Ensure that the sites meet the needs of the people we support and communicate any concerns and issues with the homes to the guardians.
  • Assist with getting to know the people supported, along with preparing the CQL assessment packets.
  • Complete satisfaction surveys with people supported every six (6) months and facilitate resolutions to issues that arise.  
  • Adhere to state and local accreditation and licensing standards and regulations as well as agency policies. 
  • Demonstrates commitment to the vision, mission, values and goals of the agency.
  • Represent Sunnyhill at various functions or events as needed. 
  • Participate in Sunnyhill committees and external professional committees as assigned by supervisor.

Requirements

 Education & Experience:

  • Bachelor’s degree in a related field or three (3 ) years of job experience in the Social Service Field.
  • Three (3) years of experience in the field of developmental disabilities.
  • Experience providing services funded through the Department of Mental Health, preferred.  

Required Skills/Abilities & Qualifications:

  • Excellent verbal and written communication skills.
  • Ability to prioritize tasks and function effectively in a high-paced environment.
  • Valid driver’s license and auto insurance (use of personal vehicle required at times).
  • Acceptable background checks.

Physical Requirements:

  • Able to stand, walk, and sit for extended periods.
  • Repetitive bending and reaching.
  • Able to lift 25 pounds from floor to waist.
