Transition Services Coordinator

Schedule/Salary/Location:

• Full-time days, on-site
• Pay Range: $20 - $22 per hour
• Located at 1111 28th St NE, Austin, MN 55912

Responsibilities:

• Participate in educational admission, transition and discharge planning of youth
• Manage and coordinate resources and activities for assigned youth to ensure that the unique educational needs and requirements are met
• Attend staff meetings and trainings
• Understand and support the Nexus philosophy and treatment modalities
• Ensure confidentiality regarding youth issues, background and treatment as well as comply with Nexus policies involving youth privacy and confidentiality rules for youth and their families
• Ensure youth safety and collaborate with co-workers to maintain an environment that is positive, respectful, clean, and therapeutic 

 Required Education and Licensure:

•  BS/BA in related field required
•  2 years’ experience working with youth and/or families
•  Valid driver’s license required. Must meet state regulating agency and Home Office driving requirements

Preferred Education and Experience:

•  4 years’ experience working with youth and/or families
