A Skilled Nursing Facility in Orange County, Florida is seeking a dynamic and empathetic Transitional Care Coordinator to join our healthcare team. The Transitional Care Coordinator is responsible for ensuring seamless transitions of care for patients as they move from one healthcare setting to another. This role requires a combination of clinical expertise, care coordination skills, and a passion for patient advocacy.

Responsibilities:

  1. Assessment and Planning: Conduct thorough assessments of patients' medical, social, and support needs to develop personalized transitional care plans. Collaborate with patients, families, and healthcare teams to identify goals and preferences for care.
  2. Care Coordination: Coordinate with healthcare providers, community resources, and support services to arrange post-discharge care, including home health services, durable medical equipment, and follow-up appointments. Facilitate communication and collaboration among interdisciplinary teams to ensure continuity of care.
  3. Patient and Family Education: Provide education to patients and families on post-discharge care instructions, medication management, red flag symptoms, and available community resources. Empower patients to actively participate in their recovery and self-care.
  4. Advocacy and Support: Advocate for patients' needs and preferences throughout the care transition process, ensuring their voices are heard and their concerns are addressed. Serve as a liaison between patients, families, and healthcare providers to resolve issues and overcome barriers to care.
  5. Documentation and Evaluation: Maintain accurate and up-to-date documentation of transitional care plans, patient education activities, coordination efforts, and outcomes. Monitor and evaluate the effectiveness of transitional care interventions, making adjustments as needed to optimize patient outcomes.

Qualifications:

  • Bachelor's degree in nursing or related field.
  • 2 years of experience in Health Care Marketing or Business Development.
  • Strong communication and interpersonal skills, with the ability to build rapport and establish trust with patients and families.
  • Excellent organizational abilities and attention to detail.
  • Knowledge of healthcare systems, care delivery models, and community resources related to transitional care.
