Service Areas: We service 25 communities in southern NH from Hooksett to Nashua.  We are the preferred provider for 3 hospitals.  

Responsibilities:

The position requires in person meetings with patients and families in a hospital as needed. Additionally, they will participate in community collaborations with hospital partners.

• Electronic medical record experience preferably with EPIC.

  Position can allow work from home, a hybrid schedule.

  Candidate must have strong customer service skills.

  Knowledge of hospice and home care, experience preferred.

  Excellent communication skills and the desire to work as part of a team.

  This is a part-time position, Weekends only.  

  
  


• Qualifications

• Graduate of an accredited school of nursing as an LPN or RN.


• Nursing Diploma or Associate degree in nursing.


• Registered as RN or LPN with New Hampshire’s Board of Nursing.


• A minimum of two years’ recent acute care experience.


• Ability to exercise initiative and independent judgment.

 Preferred Qualifications

• Bachelor’s degree in nursing.


• Community Health/Education experience.


• EMR software proficiency.

• Satisfactory criminal record.


• Satisfactory motor vehicle record.


• Bureau of Elderly and Adult (BEAS) clearance.
