General Responsibilities: 

  1. Operates as a member of the Community Services Team providing intake, screening, assessment, case planning, counseling, consultation, training, crisis intervention and case management services to DuPage County residents who are homeless or are at risk of homelessness.
  2. Prepares and maintains accurate confidential case records, contractual paperwork, status reports, and fiscal recording as required by various funders and the Standards for Excellence. This includes but is not limited to updating case notes and individualized service plans per program guidelines. 
  3. Monitor ongoing client services, advocate as needed for assigned client caseload. Provide coordination of services to assist identified population to gain access to and receive needed services.
  4. Serves as a member of the SHIFT Residential Services Team, coordinating appropriately with team to provide support to homeless families with disabilities. 
  5. Other duties as assigned within the guidelines of this position.

Essential Functions: 

  1. Provide case management services in an office or field-based setting.
  2. Following program guidelines, provides screening, comprehensive needs assessment, case planning, counseling, and case management services to families.
  3. Establish and monitor a written individualized case plan with the client and family to enable the client to live with maximum possible independence, monitoring ongoing client services, advocate as needed for assigned client caseload.
  4. Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs.
  5. Manages complex cases; works with clients who may have multiple challenges, disabilities and extended periods of homelessness. 
  6. Create and maintain confidential case records. Meet or exceed all program, funding source, and/or assessment standards related to case documentation.
  7. Complete all agency, program, and other required reporting or documentation in an accurate manner according to the required timeframes.
  8. Accurately enter and retrieve client statistical data in DuPage County’s Homeless Management Information System (HMIS) according to the HMIS Standard Operating Procedure. 
  9. Within the guidelines provided, implements and provides a range of counseling services depending on client needs (i.e. individual or family counseling, crisis intervention, budget counseling, personal hygiene, money management, job seeking skills, etc.) and provide a supportive environment to housing clients.
  10. Network, when appropriate, with the community-at-large, depending on client needs, advocating for clients and making appropriate professional contacts.
  11. Teaches families to advocate for themselves and to use community resources more effectively, empowering families to achieve their goals and direct their own lives.
  12. Works with clients, housing project manager and assistant director to identify and address each family’s housing related needs and issues.
  13. Serves as a member of the Residential Services Team, collaborating with the Y.W.C.A of Metropolitan Chicago’s employment/vocational counselor and Metropolitan Family Services’ counselors, psychiatrists and staff. 
  14. Assists with initial and ongoing volunteer mentor training, maintaining a positive working relationship with volunteer mentor parish groups, collaborating with mentors on clients’ individualized service plan and progress within the program. 
  15. Possess the ability to work independently and as part of a team, brining concerns or challenges regarding complex issues to the attention of the direct supervisor.

Requirements

  1.  A Master’s degree is required for this position, with preference in areas of social work or equivalent field. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
  2. A minimum of one (1) year of experience in social services setting is required.
  3. Experience in program specific population is preferred.
  4. Must demonstrate fundamental communication and problem-solving skills and to work on complex cases.
  5. Ability to work independently and to respond quickly and appropriately in times of crisis.
  6. Ability to relate well to individuals with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
  7. Demonstrated ability to work effectively and constructively with persons from all racial, diverse cultures, family systems, and social backgrounds.
  8. Strong verbal and written communications skills.
  9. Passage of complete background clearance and any other required testing or clearances.
  10. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
