In this role, you will play a crucial part in overseeing a panel of members with diverse health needs. Your primary responsibility will be to utilize effective case management strategies while assessing health management requirements, ensuring members receive timely and appropriate care. You will collaborate closely with healthcare providers and utilize data analytics to tailor interventions that enhance overall health outcomes. This role is vital in supporting members to achieve their personal health goals through strategic care planning and resource coordination.
Accountabilities
- Maintain oversight over a specified panel of members by performing ongoing assessment of members’ health management needs.
- Implement care management review processes consistent with established industry standards and legal requirements.
- Create individualized care plans addressing members’ needs, removing barriers to care, and identifying necessary resources.
- Document all activities ensuring compliance with regulatory requirements and accreditation standards.
- Perform additional duties as assigned.
Requirements
- High School diploma or GED required; Bachelor's Degree in Nursing preferred.
- 7 years of experience in clinical, case/utilization management, or health insurance.
- 1 year of experience in a clinical setting.
- Strong written and verbal communication skills.
- Ability to analyze data, measure outcomes, and develop action plans.
