Job Description - Transportation Coordinator

Description

To transport residents to and from medically related appointments and to assist residents with those transfers, as needed.

ACCOUNTABILITY:           Administrator

                                          Director of Nursing

DUTIES AND RESPONSIBILITIES:

  1. To transfer residents to medically related appointments as needed.
  2. Stays with patient at appointments to provide assistance upon request.
  3. To assist residents as needed with transfers in and out of the vehicle and at the appointment.
  4. Follows written and verbal before leaving facility with residents
  5. Obtain appropriate paperwork before leaving facility with resident
  6. Compiles with traffic regulations to operate vehicle in a safe and courteous manner
  7. Attend facility in-services.
  8. Understand and follow fire, safety, sanitation, blood-borne pathogen rules and Hazard Communication program.
  9. Prevent abuse, neglect and exploitation in the elderly.
  10. Promote a safe, clean environment in which the residents may live.
  11. Any other duties and responsibilities as requested by the Administrator.
  12. Become a participating facility team member with the residents as first priority.
  13. Attend facility meetings as required.

Requirements

In the event of any accident, the employee is expected to fully cooperate with the authorities.

EDUCATION:

  • CNA License
  • High School diploma or GED

EXPERIENCE AND SPECIFIC REQUIREMENTS

  • Must have an active CNA License
  • Must have current CPR Certification
  • Must possess and maintain a valid driver's license as well as a clean driving record. 
  • Must be able to push a minimum of 200 punds for a long distance. 
  • Must be able to lift bulky heavy items up to 100 pounds.
  • Must be able to speak, read, and write the English Language. 
  • Must be able to pass an MVR. 
  • Must be avaliable to work weedends on occasion.

  • Grasp and handle – pens, paperwork and small equipment
  • Sit, stand and walk
  • Twist, bend, stoop, kneel and squat
  • Fine hand motor coordination
  • Ability to read and write legibly
  • Ability to give and follow instructions
  • Ability to drive

COGNITIVE AND SENSORY REQUIREMENTS:

Talking:                Communicating with residents, families, visitors and staff   

Hearing:               To communicate on the phone effectively

                            Taking instructions from the Administrator

                            To respond to resident complaints and requests

                            To respond appropriately to disaster instructions and pages.

Sight:                   For performing job effectively and correctly

Smell:                   For accurate detection and maintenance of facility odors

SUMMARY OF OCCUPATIONAL EXPOSURE:

Tasks and procedures performed by the employee involve risks classified by C.D.C. as Category I.  (Possible direct contact with blood or other bodily fluids to which universal precautions apply, if regular duties are performed.)

SUMMARY OF OCCUPATIONAL EXPOSURE:

Must be able to tolerate a high pace as typical for a nursing facility.  Must be able to take and give instructions well and enjoy working with the elderly.  Although employee is required to sit for prolonged periods of time, he/she must also be able to tolerate prolonged standing and walking.

Must maintain good driving record. Must be covered by automobile insurance.  The facility will maintain the vehicle (gas, oil, etc.); however, the employee is responsible for parking tickets, speeding tickets or any other traffic violations.

Traffic violations could result in disciplinary action by the facility.
