Responsibilities:

• Review and verify medication orders for accuracy, appropriateness, and compliance with hospital protocols and formulary guidelines.
• Prepare and dispense medications in a timely manner, considering patient-specific factors such as age, weight, allergies, and medical history.
• Collaborate with healthcare providers to optimize medication therapy plans and resolve any medication-related issues.
• Participate in medication reconciliation processes during patient admissions, transfers, and discharges.
• Provide drug information and education to patients, caregivers, and healthcare professionals.
• Monitor patients' responses to medications and adjust therapy as necessary to achieve optimal outcomes while minimizing adverse effects.
• Utilize computerized systems and electronic health records (EHR) to document interventions and maintain accurate patient records.

Requirements:

• Inpatient hospital experience
• Active pharmacist license in the state of Michiga
