Description: Join our team as a Travel Registered Nurse (RN) in the vibrant field of home health care in Rocky Hill, CT. This position offers the opportunity to provide compassionate care to patients in the comfort of their own homes.
Responsibilities:
- Conduct comprehensive assessments and develop personalized care plans for home health patients.
- Provide skilled nursing care including wound care, medication management, and patient education.
- Collaborate with interdisciplinary team members to optimize patient outcomes.
- Coordinate care services and resources to meet patient needs.
- Ensure accurate and timely documentation of patient care activities.
Requirements:
- Active RN license in Connecticut.
- Minimum 6 months of experience as a Home Health RN.
- Familiarity with HomeCare/HomeBase EMR system.
- Current BLS certification.
- Availability for Monday through Friday day shifts.
- Ability to achieve 25-30 productivity units per week.
- Valid driver's license and reliable transportation for mileage reimbursements.
