Travel Nurse- Physical Therapist - Home Health

Job Summary:

Key Responsibilities:

• Provide physical therapy assessments and develop individualized treatment plans for patients in their homes.
• Implement therapeutic exercises and modalities to improve patients’ mobility, strength, and overall function.
• Educate patients and their families on exercises and techniques to enhance recovery and prevent further injury.
• Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
• Document patient care activities in compliance with regulatory standards and organizational policies.
• Collaborate with interdisciplinary teams to coordinate care and support patient needs.
• Travel to patients’ homes within Butte, Yuba, and Sutter Counties; mileage reimbursement is provided in addition to the bill rate.

Qualifications:

• Current state licensure as a Physical Therapist in California.
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Minimum of 1 year of home health experience preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work independently in a home setting while maintaining a strong focus on patient care.
