The Traveling / Mobile Medical Nurse Practitioner or Physician Assistant plays a critical role in delivering medical care to individuals with substance use disorders in community settings. This position focuses on providing low-barrier, trauma informed care in nontraditional environments including homeless encampments, syringe service programs, drop-in centers and mobile units. This role is crucial in supporting harm reduction efforts, reducing emergency room utilization, increasing access to care, and eliminating barriers to healthcare.
As a Traveling / Mobile Medical Nurse Practitioner or Physician Assistant with our MYTP program, you will:
Conduct community-based clinical assessments and provide direct medical care with a focus on wound care, infectious disease testing and treatment, Medication-assisted treatment evaluation and initiation.
This position collaborates closely with internal BCCS programs, and external partners to deliver comprehensive and coordinated care in the community. The NP/PA will collaborate with outreach teams to engage individuals where they are—encampments, mobile vans, and drop-in centers. They will also coordinate with case managers, peers, and partner organizations to ensure clients receive wraparound support and continuous care.
Provide health education to patients and their families, promoting informed healthcare decisions
Conduct comprehensive patient evaluations, including thorough history-taking and physical examinations. They will diagnose and treat a wide range of common and complex medical conditions, developing and implementing tailored treatment plans.
monitoring patient progress, adjusting care as needed, and providing expert contraceptive counseling and reproductive health education.
Develop care plans and refer clients to higher levels of care, including urgent care, primary care, behavioral health, and inpatient services as needed.
administer vaccinations and medications and demonstrate proficiency in phlebotomy and blood draws. Conduct and analyze appropriate laboratory and imaging studies, including X-rays, EKGs, and other relevant diagnostics.
Assist with administrative tasks as needed, maintain a clean and organized work environment, and stay current with advancements in medical knowledge and practice.
Prescribe medications, including controlled substances, in accordance with the scope of practice. This includes treatments for substance use disorders, such as Buprenorphine SL, long-acting Buprenorphine, Vivitrol, and other appropriate therapeutic options.
Knowledge, Skills, and Abilities Required of the Traveling /MobileNurse Practitioner or Physician Assistant:
Knowledgeable of current medical treatment and practices, including the treatment of substance use disorders. Practices evidence-based medicine. Skilled in diagnosing and treating substance use disorders, with knowledge of opioid agonist and antagonist therapies, including proficiency in Buprenorphine induction.
Excels in effective communication with patients, physicians, and other healthcare providers, fostering strong rapport and creating a supportive, comfortable environment for patient care.
Proficient in ordering and interpreting appropriate laboratory tests. Is skilled in diagnosing and treating substance use disorders and common acute and chronic medical conditions. Is proficient in the treatment of STIs, Hepatitis C, HIV, among others. Is comfortable providing women's services, including prescribing birth control.
Uses up-to-date techniques, principles, and practices for delivering medical evaluation, diagnosis, and treatment in individuals with substance use disorders.
Strong knowledge of medical terminology and procedures
Requirements
Qualifications for this position are:
REQUIRED: Graduation from an accredited Physician Assistant or Nurse Practitioner program
REQUIRED: Certified Physician Assistant, PA-C or Certified Registered Nurse Practitioner, CRNP, and licensed to practice in the state of Delaware
REQUIRED: 2+ years of family medicine or internal medicine
PREFERRED: 1 year experience in addiction medicine
REQUIRED: BLS and ACLS certifications
REQUIRED: DEA License
Pay:
$125,000 - $150,000
Schedule:
Monday – Friday
8:30 a.m. - 4:30 p.m.
