The Grand Rehabilitation & Nursing at Barnwell is seeking a detail-oriented and skilled Treatment Nurse (LPN) to join our nursing team. In this role, you will be responsible for delivering high-quality resident care through precise completion of physician-ordered treatments, ongoing assessment and monitoring, and thorough documentation.
The Treatment Nurse ensures proper wound care, skin integrity management, accurate treatment implementation, and close collaboration with the interdisciplinary care team to support optimal resident outcomes.

Obtain and record vital signs for residents on the assigned unit.
Perform physician-ordered treatments and wound care following protocols and infection control standards.
Complete finger-stick blood glucose checks and administer insulin as ordered.
Assist with monthly and as-needed resident weights.
Observe, report, and document any changes in resident condition to nursing leadership or the physician.
Maintain clean, organized, and fully stocked treatment carts.
Collaborate with the interdisciplinary team to support individualized care plans.
Identify residents at risk for skin breakdown and implement preventative strategies.
Follow universal precautions and infection-control procedures at all times.
Accurately document all treatments, interventions, and resident observations in the EMR.
Complete daily treatment logs and ensure physician orders are carried out as written.
Maintain up-to-date treatment records and make timely updates when new orders are issued.
Attend daily morning report to exchange clinical information and updates.
Communicate effectively with CNAs, nurses, physicians, unit managers, administrators, and other team members.
Participate in skin rounds, care plan meetings, and quality assurance activities as needed.
Assist in staff education related to treatments, skin integrity, and infection prevention.
Ensure compliance with facility policies, state regulations, and federal guidelines.
Perform other duties as assigned by Unit Managers, ADONs, or the Director of Nursing.
Current, valid LPN license in New York State
Prior experience in long-term care, wound care, or treatment nursing preferred
Strong assessment, organizational, and documentation skills
Ability to work independently and effectively within a team
Basic computer proficiency and experience with EMR systems
