Job Description - Triage Nurse


The Telephone Triage Nurse plays a critical role in independently planning and providing professional nursing care to patients through telecommunications technology. This position focuses on delivering, managing, and coordinating care in alignment with established nursing standards and practices. The ideal candidate will demonstrate strong clinical and communication skills, ensuring quality patient-centered care while collaborating with multidisciplinary teams. 


Requirements

Patient Care Management

· Assess patient symptoms and needs through effective telephone communication.

· Provide medical advice based on clinical protocols and guidelines.

· Determine the urgency of patient conditions and recommend appropriate care levels, ensuring timely responses and interventions.

Care Coordination

· Deliver, manage, and coordinate care and services in accordance with nursing standards and policies.

· Collaborate with healthcare teams, including doctors and other medical professionals, to ensure continuity and quality of patient care.

· Ensure proper scheduling and follow-up on test results for providers.

Leadership and Team Collaboration

· Direct and lead assigned team members while fostering a collaborative environment

· Participate in professional development and maintain required competencies.

Documentation and Compliance

· Document patient interactions, assessments, and recommendations accurately in electronic medical record systems.

· Maintain patient confidentiality while adhering to ethical and legal standards, including HIPAA regulations.

Patient Education and Support

· Educate patients on managing their health conditions at home and provide emotional support and reassurance to patients and their families.

· Utilize active listening skills and apply critical reasoning to explain test results and medications to patients.

Emergency Call Management

· Manage emergency calls effectively, providing immediate guidance for critical situations.

· Monitor and reassess patients, ensuring appropriate follow-up care and referrals.

Continuous Improvement

· Stay up to date with current medical practices, guidelines, and regulations.

· Participate in regular training and continuing education to enhance knowledge and skills.

  

SKILLS   / QUALIFICATIONS / REQUIREMENTS

Required Skills

· Excellent verbal and written communication skills for effective interaction with patients, caregivers, and healthcare professionals.

· Strong critical thinking and decision-making abilities to assess patient needs rapidly.

· Ability to manage multiple tasks efficiently in a fast-paced environment.

· Attention to detail in gathering and documenting patient information accurately.

· Proficiency in using medical software and electronic health record systems.

· Empathy and compassion to provide emotional support to patients and families.

· Strong organizational skills and capability to work both independently and collaboratively.

Qualifications

· Current RN license in the State of Nebraska.

· Basic Cardiac Life Support (BCLS) certification required.

· Graduation from an accredited School of Nursing; BSN preferred.

· Previous experience in Telephone Triage and knowledge of Health Insurance preferred.

· Strong understanding of medical terminology, procedures, and medications.

· Proficient computer skills and comfort with telehealth technologies. Knowledge of medical laws and regulations, including HIPAA and patient privacy laws.
