Job Description - TSS Program Coordinator

Department: Senior Housing Services (SHS) 


Status: Full-time, non-exempt 


Hours: Monday –Friday 8:30 am- 5:00 pm. Occasional nights or weekends 


Compensation: $32.00 per hour
Reports To: Senior Housing Services Manager 




About Us 


St. Mary's Center is a community of hope, healing and justice dedicated to improving the well-being of Seniors and Preschool Families in West Oakland by meeting basic needs for food, housing, and social connection. We build on a strong foundation of direct service provision and participant-led advocacy that amplifies the voices of people directly impacted by hunger, homelessness, and inequity to create a more just and caring society.  


 


Today, our staff of 40 provides Senior Homeless and Housing Services, including case management and housing clinics; transitional housing at multiple sites; and a vibrant Community Center offering hot meals, social connection, and leadership development. We are partnered with a local nonprofit developer to build and service 73 new homes for Seniors near our headquarters. 


 


Position Summary 


The TSS Program Coordinator provides Tenancy Sustaining Services designed to help older adults maintain housing stability, strengthen independence, and prevent returns to homelessness. 


The position receives participants through a formal handoff from Housing Navigation or Transitional Housing and serves as the primary coordinator for housing retention services, community integration, benefits linkage, and long-term housing stability. 


Target caseload is 30–35 active participants. 




Essential Duties and Responsibilities 


Housing Retention & Stability Services (35%) 



  • Develop and maintain Housing Support Plans. 



  • Conduct home visits and participant check-ins. 



  • Monitor housing stability and tenancy concerns. 



  • Assist participants in maintaining lease compliance. 



  • Identify and address barriers to housing retention. 



  • Coordinate housing crisis prevention activities and critical time interventions. 




Care Coordination & Benefits Linkage (20%) 



  • Coordinate access to healthcare, behavioral health, and supportive services. 



  • Facilitate referrals to ECM, IHSS, CHW, transportation, and community-based services. 



  • Assist participants with benefit maintenance and renewal activities including Medical, SNAP, and others. 



  • Coordinate service plans with community partners. 




Housing Transition Support (15%) 



  • Receive participant handoff from Housing Navigation or Housing Pathways. 



  • Coordinate move-in support activities, including ordering furniture.  



  • Coordinate furniture delivery and household setup services. 



  • Assist participants with housing orientation and adjustment. 




Community Building & Participant Engagement (15%) 



  • Facilitate monthly housing stability classes or workshops for housed participants. 



  • Promote social connection and community engagement. 



  • Support participant leadership and peer engagement opportunities. 



  • Coordinate housing graduation recognition activities. 




Landlord Relations & Housing Advocacy (5%) 



  • Communicate with landlords when tenancy issues arise. 



  • Support conflict resolution and housing advocacy efforts. 



  • Assist participants in navigating housing-related concerns. 




Documentation & Compliance (10%) 



  • Maintain HMIS and Salesforce documentation in a timely fashion. 



  • Complete required Housing Support Plans and service documentation. 



  • Track participant outcomes and housing retention indicators. 



  • Participate in quality assurance activities. 




Performance Expectations 



  • Housing retention goals achieved. 



  • Home visits completed timely. 



  • Monthly participant engagement activities conducted. 



  • Housing Support Plans maintained. 



  • Documentation standards achieved. 



  • Participants remain successfully housed. 


 


Minimum Qualifications 



  • Bachelor’s degree in Human Services, Social Work, Public Health, or related field preferred, or three years’ experience in homeless or residential services 



  • Two years of experience in housing services, case management, supportive services, or homelessness services. 



  • Experience working with older adults or vulnerable populations. 



  • Valid California Driver’s License. 




Preferred Qualifications 



  • Experience with Housing First, CalAIM, and supportive housing programs. 



  • Experience conducting home visits and housing retention services. 



  • Experience with HMIS and Salesforce.

  • Ability to use computers, phones, tablets, and related technology for documentation and communication.
