Job Description - Unit Clerk

Description

Introduction: 

Under the supervision of the Nurse Manager, the Unit Clerk is responsible for the coordination of health information for all patients on the respite unit. This includes the transcription and processing of orders on the patient chart, scheduling medical appointments, coordinating patient transportation, and obtaining and managing patient health records. 

Essential Functions: 

  1. Demonstrates a commitment to the mission and values of the facility to preserve the dignity and confidentiality of every person through courteous behavior and respectful communications with all patients, Kairos members, co-workers and visitors. 
  2. Initiates patient records on admission and enters all pertinent information into the patient chart and electronic record. 
  3. Transcribes and processes orders from providers, nurses, and case management in the patient chart efficiently and accurately. 
  4. Schedules patients for primary care, specialist appointments, and/or procedures. When appropriate, assists patients to do their own scheduling.
  5. Obtains referrals and authorizations for appointments when necessary and facilitates completion of appropriate forms for appointments. 
  6. Coordinates daily transportation schedule and facilitates communication between patient(s) and driver. 
  7. Maintains accurate and updated daily Nursing Care Worksheets. 
  8. Schedules patient labs as ordered by provider. 
  9. Answers telephones and communicates messages professionally and in a timely manner to patients and staff as needed. 
  10. Obtains patient authorization for medical record requests. 
  11. Communicates with patients about their next day appointments. 
  12. Solicits, files, and scans reports of recent labs, procedures, and office/clinic visits from other healthcare providers. 
  13. Disassembles patient charts on discharge, ensuring all records are included, accurate, and complete. 
  14. Maintains a clean and confidential work area and ensures adequate forms and office supplies for the Nurses' Station.
  15. Performs basic data entry, including maintaining the patient log. 
  16. Performs other duties as requested. 

Requirements

Education: 

  • Bachelor's degree. 

Experience: 

  • Experience working with individuals who are experiencing homelessness is preferred. 

Skills: 

  • Proficiency with Microsoft Office Suite required. 
  • Must have strong organizational skills, attention to detail, and an ability to multi-task. 
  • Must be able to communicate effectively, verbally, and in writing.
