Job Summary:

Charter Oak Health Center is seeking An Urgent Care Nurse Practitioner (APRN) to join our team.  In this role, you will be responsible for providing high-quality, immediate care to patients with non-life-threatening conditions. This role involves diagnosing and treating a variety of illnesses and injuries, performing minor procedures, and collaborating with other healthcare professionals to ensure comprehensive patient care.

Key Responsibilities:

• Conduct thorough assessments of patients' health conditions, including medical history and physical examinations.
• Diagnose acute illnesses and injuries, develop treatment plans, and prescribe medications as needed.
• Perform minor medical procedures such as suturing, wound care, and splinting.
• Order, interpret, and follow up on diagnostic tests such as X-rays, EKGs, and lab work.
• Educate patients and their families on treatment plans, preventive care, and healthy lifestyle choices.
• Work closely with physicians, nurses, and other healthcare professionals to provide coordinated care.
• Maintain accurate and detailed patient records, including treatment plans and progress notes.
• Provide immediate care in emergency situations and stabilize patients for transfer if necessary.

Requirements

Qualifications:

• Master's degree in nursing (MSN) 
• 1 to 2 years' experience in ambulatory care setting preferred, however new graduates are welcome to apply.
• Active Nurse Practitioner (NP) license and certification as an Advanced Practice Registered Nurse (APRN) in the State of CT.
• Strong clinical and diagnostic skills.
• Excellent communication and interpersonal skills.
• Proficiency in using electronic health records (EHR) systems.
• Certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
