What You'll Do

As a Value Based Care Pharmacy Technician, you’ll support our mission by helping patients stay on track with their medications—directly impacting our CMS Star Ratings through improved medication adherence. You'll work closely with pharmacists and healthcare teams to ensure safe, efficient, and cost-effective pharmaceutical care.

Key Responsibilities

• Lead efforts to meet HEDIS Medication Adherence measures.


• Collaborate with Revere Health pharmacists and the Population Health team.


• Conduct proactive patient outreach to promote medication compliance and resolve barriers to adherence.


• Answer patient inquiries, manage prescription refills, and update medication records.


• Communicate with providers and care teams via phone and internal messaging systems.


• Liaise with outside vendors, pharmacies, and insurance providers while professionally representing Revere Health.


• Maintain a clean and safe work environment in compliance with regulations.


• Adhere to HIPAA and infection-control policies.


• Accurately maintain records and document prescription activity.


• Generate reports by collecting and summarizing key data.


• Support team efforts and contribute to continuous improvement.

What We’re Looking For

• Certified Pharmacy Technician with a current license.


• Solid understanding of state and federal pharmacy regulations.


• Familiarity with HEDIS Medication Adherence metrics preferred.


• Experience in healthcare or a fast-paced customer service role.


• Proficient with computers and electronic health records (EHRs).


• Strong communication skills and a proactive mindset.
