We’re looking for a Registered Nurse (RN) Care Coordinator to join our innovative Value-Based Care team. In this role, you’ll serve as a key clinical partner—navigating patients to the right care, at the right time, in the right setting.


About the Role


As an RN Care Coordinator, you’ll be key to our VBC strategy—triaging incoming patient needs, guiding them through their care journey, and partnering closely with Physicians, Value Based Care Navigators, and Specialists. Your decisions will directly impact patient outcomes, healthcare utilization, and overall care experience.


What You’ll Do


🔍 Clinical Triage & Assessment



  • Manage inbound and outbound patient communications (calls, messages, referrals) with empathy and urgency.

  • Perform virtual assessments to evaluate symptoms, risks, and care needs.

  • Use clinical protocols and critical thinking to determine appropriate levels of care.


🧭 Care Navigation



  • Clearly communicate care plans and next steps to patients.

  • Coordinate referrals, appointments, or home health services as needed.

  • Serve as a trusted clinical guide, helping patients navigate complex systems.


🤝 Team Coordination



  • Collaborate with PCPs, Specialists, and Value Based Care Navigators for seamless care transitions.

  • Contribute clinical insights to care planning in interdisciplinary team meetings.

  • Document assessments and interventions with precision in the EHR.


📊 Value-Based Care & Quality



  • Support goals around reducing ED visits, enhancing HEDIS/Stars scores, and improving overall care efficiency.

  • Help evolve triage protocols and navigation workflows to better serve patients.


🧠 Patient Education



  • Deliver clear, compassionate education to empower patients in their health decisions.


What We’re Looking For


📚 Education:



  • Required: Associate Degree in Nursing (ADN)

  • Preferred: Bachelor of Science in Nursing (BSN)


📋 Licensure/Certifications:



  • Required: Active RN license (State of Utah)

  • Required: BLS Certification


🩺 Experience:



  • Required: 1 year of direct patient care experience (e.g., ambulatory, acute, ED, urgent care, or home health)

  • Preferred: Experience in care coordination, triage, case management, or value-based care


💡 Key Skills:



  • Strong clinical judgment, triage, and decision-making skills

  • Excellent communication and patient education abilities

  • Proficiency in EHR systems and Microsoft Office

  • Knowledge of VBC principles and population health strategies

  • Ability to thrive in a fast-paced, collaborative environment
