Job Description - Vascular Program Coordinator

Position Summary

  • The Vascular Program Coordinator is responsible for supporting the growth, coordination, quality, and operational success of the vascular program across the continuum of care. This role serves as a clinical and operational liaison between physicians, patients, interdisciplinary departments, and referring providers to ensure timely, evidence-based, and patient-centered vascular care.
  • The coordinator supports program development initiatives, quality improvement activities, patient navigation, community outreach, data management, and multidisciplinary collaboration of vascular and related cardiovascular service lines.

Principal Accountabilities

Education

  • Registered Nurse (RN), Registered Radiologic Technologist (RT), or Registered Vascular Technologist (RVT/RVS) required
  • Bachelor’s degree preferred
  • Minimum of 3 years of cardiovascular, vascular, critical care, interventional, or related clinical experience preferred

Licenses and Certification

  • Registered Nurse (RN), Registered Radiologic Technologist (RT), or Registered Vascular Technologist (RVT/RVS) required

Experience and Skills

  • Minimum of 3 years of cardiovascular, vascular, critical care, interventional, or related clinical experience preferred
  • Strong organizational, communication, and interdisciplinary collaboration skills
  • Experience with EPIC and clinical data reporting preferred
  • Knowledge of vascular disease processes, quality metrics, and evidence-based care preferred

Physical Activities

  • Intermittent hand manipulation required
  • Intermittent lifting and carrying of 50 pounds

Role Specific Responsibilities

  • Program Coordination & Patient Navigation
  • Coordinate and streamline vascular patient care across multiple specialties including podiatry, wound care, infectious disease, cardiology, primary care, and diagnostic imaging.
  • Serve as a primary point of contact for vascular patients and families throughout the continuum of care.
  • Facilitate timely referral management, follow-up coordination, and patient scheduling support.
  • Identify potential vascular patients through EPIC-generated reports and screening opportunities
  • Collaborate with the transfer center, house supervisors, and physicians to support incoming patient transfers and facilitate timely communication and care readiness.
  • Assist with coordination of follow-up care, discharge planning, and transitions between inpatient and outpatient settings.
  • Communication & Outreach
  • Maintain effective communication with physicians, interdisciplinary care teams, and referring providers to support continuity of care.
  • Serve as a liaison between the vascular program and internal/external stakeholders.
  • Support community outreach initiatives, health fairs, screening events, and educational programs related to vascular disease prevention and awareness.
  • Assist with vascular preventive care campaigns.
  • Develop, maintain, and update patient education materials, provider resources, and program marketing materials.
  • Promote positive referring provider relationships through consistent communication and follow-up.
  • Education & Clinical Liaison Responsibilities
  • Provide patient and family education related to vascular diagnoses, procedures, medications, discharge instructions, and follow-up care.
  • Support interdisciplinary care planning and communication among physicians, nursing, ancillary departments, and outpatient services.
  • Participate in professional development activities, conferences, and service line committees related to vascular and cardiovascular care.
  • Data Management & Quality Monitoring
  • Collect, track, analyze, and report program data including procedural volumes, patient outcomes, readmissions, repeat procedures, and length of stay metrics.
  • Support data abstraction, registry participation, and reporting activities for programs such as the PERT Registry and Vascular Quality Initiative (VQI).
  • Prepare reports and outcome summaries for vascular, invasive cardiovascular, pulmonary embolism, and CLI committees/councils.
  • Monitor quality metrics and identify opportunities for process improvement and program growth.
  • Assist with preparation of dashboards, presentations, and performance reports for leadership and physician stakeholders.
  • Administrative & Strategic Support
  • Maintain and update clinical protocols, care pathways, order sets, and educational materials to align with current evidence-based guidelines.
  • Assist with development and implementation of vascular clinical algorithms and standardized care pathways to improve patient outcomes and operational efficiency.
  • Participate in vascular program meetings, multidisciplinary conferences, and quality improvement initiatives.
  • Coordinate distribution of care plan communication to primary care providers and referring teams as appropriate.
  • Submit and maintain physician call schedules and other program-related administrative documentation.
  • Serve as a dyad partner supporting strategic, operational, and quality initiatives for the vascular program and related cardiovascular service lines.

Compensation (Commensurate with experience):

$70,200.00 - $108,804.80
