Position Overview
Do you know how to earn trust in the places where Veterans live, gather, and seek support? The Transition House, Inc. is seeking a relationship-builder who can take our Veteran Connection and Suicide Prevention Program beyond office walls and into the communities where timely outreach matters most.
As the Veteran Community Outreach and Reentry Specialist, you will find and engage Veterans who may otherwise be missed, strengthen referral relationships across Orange and Osceola Counties, and connect eligible Veterans with the SSG Fox team. Justice-involved Veterans are an important focus, but this is not solely a reentry role. You will also reach Veterans who are homeless or unstably housed, disconnected from VA care, isolated, unemployed, or facing behavioral health, substance-use, financial, family, or housing stress.

What You Will Do
Reach Veterans and Build Trust
- Conduct direct outreach in community settings and follow up with Veterans who may need more than one contact before they are ready to engage.
- Attend Veteran events, community meetings, reentry events, outreach activities, and resource fairs throughout the service area.
- Engage Veterans affected by housing instability, justice involvement, isolation, unemployment, disconnection from VA care, or other significant life stressors.
Build Strong Community Partnerships
- Develop relationships with Veteran organizations, shelters, hospitals, behavioral health providers, county agencies, housing providers, jails, correctional and reentry programs, and other community partners.
- Provide presentations and referral education to TTHI teams and community partners so they understand who the program serves and how to connect Veterans with support.
Connect Veterans to Timely Support
- Conduct initial SSG Fox eligibility screening and, after completing required training and competency validation, administer VA-required suicide-risk and baseline mental-health or wellbeing screening.
- Take immediate action when a Veteran presents with elevated or imminent suicide risk, including same-day warm handoffs to VA suicide-prevention resources, the Veterans Crisis Line or 988, crisis services, or emergency supports.
- Connect eligible Veterans immediately with Case Manager and Veteran Navigators and the rest of the SSG Fox team.
Track Outreach and Strengthen Engagement
- Track outreach contacts, referrals, screenings, warm handoffs, and successful engagement in accordance with grant requirements.
- Share referral trends, engagement barriers, and community needs with the program team to support responsive outreach and service coordination.
What You Bring
Education and Experience
- Experience in Veteran services, community outreach, homelessness, reentry, behavioral health, human services, or related community work.
- A bachelor's degree in a related field is preferred, but significant relevant experience may substitute.
- Experience with Veterans, jails/corrections, reentry, homelessness systems, or community behavioral health is strongly preferred.
Knowledge Skills and Abilities
- Strong interpersonal, communication, presentation, and relationship-building skills.
- Comfort initiating conversations, representing the program in the community, and following up consistently.
- Ability to work independently in the field while coordinating closely with the Program Director and SSG Fox team.
- Sound judgment and the ability to follow established screening, documentation, escalation, and warm-handoff procedures.
- Ability to engage Veterans and community partners with professionalism, respect, and sensitivity to individual circumstances.
Licenses and Travel
- Valid driver's license and ability to travel regularly throughout Orange and Osceola Counties and the surrounding service area.
Preferred Background
We especially welcome Veterans and candidates with direct experience supporting Veterans, working in Veteran-serving programs, or navigating VA, homelessness, behavioral-health, corrections, or reentry systems. Veteran status is not required.
Required Training and Competencies
The selected candidate must complete required training and demonstrate competency in:
- SSG Fox eligibility requirements and approved program services.
- VA-required suicide-risk and baseline mental-health or wellbeing screening tools.
- Suicide warning signs, participant-safety procedures, crisis escalation, and same-day response expectations.
- Veterans Crisis Line and 988 procedures, VA suicide-prevention referrals, and warm handoffs.
- Outreach, referral, screening, engagement, confidentiality, and grant-documentation requirements.
Work Environment
This position is community-based and includes office, partner, and field outreach responsibilities throughout Osceola and Orange Counties. The role involves regular interaction with Veterans, families, community providers, correctional and reentry partners, VA contacts, and TTHI team members.
Schedule Expectations
This position generally operates during standard business hours. Outreach needs may occasionally require schedule flexibility, including evenings or weekends, for Veteran events, resource fairs, community meetings, urgent participant-safety matters, training, or other time-sensitive responsibilities.
