has exclusively retained Kirby Bates Associates to identify their new

VP Care Management, Post-Acute Care Transition (VP)

to provide executive leadership for Care Management (CM), Discharge Planning, and Post Acute Care Transitions across the continuum.

The VP will have responsibility across Kaleida Health System which includes Buffalo General Medical Center/Gates Vascular Institute, DeGraff Medical Park, Millard Fillmore Suburban Hospital, John R. Oishei Children's Hospital, Bradford Regional Medical Center and Olean General Hospital, as well as Great Lakes Integrated Network, HighPointe and DeGraff skilled nursing facilities, associated partners across the system, and community organizations.

Kaleida Health also offers home health care through the Visiting Nursing Association of WNY, Inc. In addition, Kaleida Health’s hospitals support residency training programs of the

University at Buffalo Jacobs School of Medicine and Biomedical Sciences , training hundreds of residents each year.

Salary $190-220K plus bonus.

Qualifications:
Bachelor’s degree in nursing required, Master’s degree in Health or Business preferred.
10 years of care management/utilization management experience in a hospital or ambulatory setting.
7 years of progressive leadership experience.
Case Management Certification (ACM, CCM, HCQM, CMAC) preferred.
Leadership experience within a union environment preferred.
