Job Description - Vocational Case Manager of Job Supports


Community Living, Inc. is hiring a Vocational Case Manager to support individuals with developmental and intellectual disabilities in maintaining meaningful employment. In this role, you’ll provide one-on-one coaching, track progress on employment goals, and partner with employers to ensure participants have the tools and support they need to succeed. This is a great opportunity to make a lasting impact by helping people build confidence, independence, and long-term success in the workplace.

Location: St. Charles County
Status: Full-Time
Pay: $23.25/hour

What You’ll Do

  • Provide individualized job coaching to help participants strengthen hard and soft skills.

  • Develop and implement coaching plans, job checklists, and task analyses.

  • Conduct retention visits to monitor progress and address challenges quickly.

  • Coordinate with employers, participants, and Employment Services staff to support workplace success.

  • Maintain accurate documentation of participant progress and prepare reports for stakeholders.

  • Ensure services meet all funder, state, and accreditation requirements.

  • Build and maintain positive relationships with employers and community partners.

What We’re Looking For

  • High school diploma or GED required; bachelor’s degree in social services or related field preferred.

  • Experience working with individuals with intellectual or developmental disabilities preferred.

  • Strong communication, problem-solving, and organizational skills.

  • Ability to manage multiple caseloads and keep accurate records.

  • Proficient with Microsoft Office (Outlook, Word, Excel).

  • Valid driver’s license and reliable transportation required.

  • Flexible schedule availability, which may include evenings, weekends, or split shifts.
