LEVEL: IV  

JOB TITLE: Voice Center Youth Outreach Coordinator  

PROGRAM AREA: Crisis to Housing 

JOB OBJECTIVE: To actively participate in the crisis to housing continuum at Oasis. Provide leadership and operations oversight at the micro drop-in center at the MDHA Vision Center supporting, youth, ages 10-17, by staying connected to the Metro school system and other crucial resources. 

Direct Service

Essential Functions: 

  • Provide supervision and support to program case manager. 
  • Oversee and coordinate day-to-day operations of the Vision Center micro drop-in center for youth ages 10-17, including monthly specialty programming. 
  • Conduct outreach to the neighboring MDHA properties (Napier Place & Sudekum Apartments), including attending community events that fall within operating hours. 
  • Establish relationships with community partners for appropriate referrals when needed.
  • Develop engagement strategies, outreach content, and coordinate programming associated with the needs of the youth accessing the center. 
  • Identify youth who qualify for educational programs and assist eligible clients in applying for publicly sponsored head start programs, summer lunch programs and other city-wide youth and scholarship programs, providing necessary follow-up, tracking, and data to demonstrate continuous enrollment.
  • Coordinate with other Oasis Programs and appropriate external programs for support services and referrals.
  • Regularly communicate with Metro Nashville Public Schools and the HERO Program for care coordination, programming, and referral resources. 
  • Actively manage, analyze, and assess program progress on meeting agreed upon targets. Use data to make decisions and course correct as necessary.  
  • Other related duties as assigned to support organizational goals and objectives and youth-centered care.
  • Provide general customer service to all MDHA Resident Services' clients, including but not limited to providing information and referral, enrollment information and emergency referral response. Should maintain some knowledge of departmental policies and procedures.
  • Apply and integrate relevant strategies and guiding principles of Nashville’s Coordinated Community Plan to Prevent and End Youth Homelessness. 
  • Represent Oasis Center in the community to educate about issues of RHSY.
  • Transport youth as needed. 

Administration

Essential Functions:

  • Document client interactions in a timely, consistent and ethical manner. 
  • Attend community meetings for networking and resource updates as needed. 
  • Attend other agency networking/coordination meetings as needed. 
  • Effectively capture data and track outcomes in agency database and HMIS (as needed). 
  • Maintain consistent and timely records, including relevant reports to meet funding and licensing requirements and document program spending. 
  • Share program data with MDHA staff as requested.
  • Conduct follow-up contacts, as needed. 
  • Participate in staff and program development by attending trainings and giving feedback. 
  • Attend weekly supervision, weekly team meeting and monthly all-staff meeting.

Requirements

Minimal Requirements / Performance Standards:

  • Ability to effectively communicate in oral and written form. 
  • Ability to monitor client interaction visually and auditorily. 
  • Ability to form strong, caring and affirming relationships with all youth – specifically including LGBTQ+ youth. 
  • Ability to apply Motivational Interviewing principals and operate from a Trauma Informed Care approach.  
  • Ability to work from an anti-oppression, strengths-based model with an understanding of racial inequities and other forms of discrimination for youth in care. 
  • Knowledge and ability to access community resources. 
  • Knowledge and ability to apply Positive Youth Development approaches and an understanding of adolescent development.  
  • Ability to deliver services through a low barrier approach. 
  • Valid driver’s license and good driving record. 
  • One-year relevant experience 

Educational Requirements: Bachelor’s degree in a human service field or 4 years relevant experience.
