We have a fantastic opportunity for a Licensed Practical Nurse to join our team! This role is a vital part in the care of our residents and and to be a valued member of our community! We look forward to learning more about how, together, we can Transform the Perception of Age!

The Licensed Practical Nurse we are hiring for will:

• Work every weekend, 8:00am to 4:00pm (daytime)


• Work three weekdays (Monday, Tuesday, Wednesday, Thursday and/or Friday), 10:00pm to 6:00am (overnight) 

Our Licensed Practical Nurses:

• Provide compassionate and effective care and treatments to our residents. 


• Work with a team of professionals to ensure the residents comprehensive needs are met.


• Review routines, calendar of events and resident needs


• Ensure medications and supplies are ordered, in stock and counted


• Ensure all charting and record keeping is accurate and complete


• Attend meetings, trainings


• Promote the Mission, Vision and Values of Transforming Age, our Culture of Excellence, in all areas of responsibility.


• Other duties as assigned.

What you need to succeed!

• Be a graduate of an accredited LPN Program


• Have an active LPN license in the state, in good standing


• Experience in a Nursing Home (Assisted Living or Long-Term Care preferred, but not required)


• Have flexibility, able to work the occasional holiday as needed.


• Have knowledge of DHS Regulations


• Possess excellent interpersonal and communication skills and the ability to work well and collaboratively with team members and external individuals. 

Physical Requirements:

· On occasion, may need to push/pull/support up to 75lbs.
